Abstract

INTRODUCTION: Heart Failure (HF) is the inability of the heart to maintain an output adequate to meet the metabolic demands of the body. The prevalence of heart failure in adult population is increasing, numerous prognostic biomarkers for hospitalisation and/or death in HF patients have been researched and found. Since it is difficult to stratify the risk of HF patients, their clinical applications are still limited. Red cell distribution width (RDW) has recently been discovered to be a novel prognostic marker in patients with heart failure. RDW is reported alongside complete blood count, no additional cost is incurred in knowing it. There are very few of published reports regarding relationship between RDW and Heart Failure in India. Hence, this study is done to assess the relationship between RDW and heart failure. AIM OF THE STUDY: To estimate RDW among patients with heart failure. To determine the relationship between RDW and severity of heart failure (NYHA functional class and LV ejection fraction). STUDY DESIGN: Study Duration: 18 months. METHODOLOGY: 100 representative cases of heart failure which satisfied the criteria were selected. A detailed history was obtained from them and symptom analysis was done. A detailed clinical examination, CBC, RDW, ECG & ECHO were also done. RESULT: The RDW in patients with LVEF <30% was 58.8 fl. The patients with NYHA-4 had RDW 61.6 fl. CONCULSION: To conclude, in our study of 100 Heart failure patients, we found that commonly encountered clinical symptoms with which they presented included orthopnea followed by edema. The most commonly seen NYHA class was Class III severity, and the mortality rate was 12%. We also observed that there was a statistically significant association seen with respect to RDW distribution and disease severity, aetiology, comorbidity, and mortality among heart failure cases. In particular, a 10% greater risk of all-cause mortality was linked to a 1% rise in baseline. Red cell distribution width is a readily available test. As it is reported alongside a complete blood count, no additional cost is incurred in knowing it. It is a good marker with good prognostic value.
