Abstract

INTRODUCTION: Thrombocytopenia is a common occurrence in pregnancy. It is the second most common hematological finding in pregnancy next to anemia. Pregnancy is associated with numerous physiological and pathological changes in platelet number and its functions and which can be of clinical concern. Inherited qualitative and quantitative platelet disorders may also manifest during pregnancy with the risk of bleeding. OBJECTIVE: The purpose of this study is to determine the various causes, prevalence of thrombocytopenia in pregnancy. Diagnosis, evaluation and management of thrombocytopenia at prompt time decreases the maternal mortality and morbidity. And to evaluate the fetal outcome in mothers with thrombocytopenia. MATERIALS AND METHODS: This is a prospective analysis of the Inpatients of antenatal mothers with thrombocytopenia in third trimester who admitted in Raja Mirasudhar hospital for a duration of 1 year. The prevalence and the etiology of thrombocytopenia was analysed and the maternal morbidity, mortality and the outcome of pregnancy were observed. The neonatal outcome was evaluated. RESULTS: Of the total 8544 mothers in third trimester 273 (3.1%) found to have thrombocytopenia, the most common etiology is Gestational thrombocytopenia (69.9%) and the next being hypertensive disorder (25.27%) and ITP 3 (0.01%). There was no influence on Age, Gravida and parity and Gestational Age. There was no influence of thrombocytopenia on mode of delivery. Haemorrhagic manifestations occurred in 6.2% (16) patients majority 64.7% intra partum events, mainly PPH. Blood and blood products transfusion mainly with patients with HELLP and severe pre eclampsia. Neonatal mortality was 4.02% (11) mainly due to prematurity,and NICU admissions 18.31% (50). Neonatal thrombocytopenia reported in 5.8% (16) and persistant in 1.09% (3). Normalisation of platelet count usually occurred before 10th PND. CONCLUSION: The prevalence of thrombocytopenia in pregnancy is 3.1% in third trimester in patient attending RMH. The most common cause of thrombocytopenia being Gestational Thrombocytopenia and next being hypertensive disease complicating pregnancy. Thrombocytopenia has no influence on age, parity, gestational age, mode of delivery.
