Abstract

BACKGROUND: Psoriasis is a common and chronic inflammatory, immune-mediated skin disease. In the past psoriasis has been regarded as a disease affecting only the skin and joints. The prevalence rates of cardiovascular risk factors including hypertension, diabetes mellitus, dyslipidemia, obesity, and metabolic syndrome is found to be increased in patients with psoriasis. As a result, mortality rates has become higher and life expectancy shortened in patients with psoriasis when compared to the general population. Hence it is important to be aware of this increased risk and to detect modifiable risk factors early and to refer patients to other specialists when needed for the prevention of major cardiovascular events. This study aims to define the epidemiological, clinical, laboratory profile and the cardiovascular risk based on the Framingham risk score of patients seen at a tertiary care center at Chengalpattu. AIM OF THE STUDY: 1. To estimate the prevalence of cardiovascular risk factors in patients with psoriasis. 2. To find the risk of developing coronary artery disease using the Framingham risk score. METHODOLOGY: It is a cross-sectional study conducted at the Department of Dermato-Venereo-Leprology at Chengalpattu Medical College for a period of one year. A total of 110 patients who fulfilled the inclusion criteria were recruited for the study. Patient's data such as age, sex, smoking pattern, alcohol intake, history of hypertension, type 2 diabetes, dyslipidemia, cardiovascular events (angina, MI, stroke,) and treatment for the same, duration of the diseaseand PASI score was recorded.Patient’s height, weight, waist circumference, blood pressure were measured. Laboratory investigations like FBS, total cholesterol, LDL, HDL, triglycerides, ECG and Echocardiogram was done. Metabolic syndrome and the 10 year risk of coronary artery disease was assessed using Framingham risk score. RESULTS: A total of 110 patients of plaque type psoriasis were included in the study. 78.18% patients had atleast one risk factor for cardiovascular disease. The most common risk factor was dyslipidemia (increased triglycerides, 49.8% and decreased HDL cholesterol 49.1%) followed by hypertension (41.8%), obesity(30.9%), metabolic syndrome(39.1%) and diabetes mellitus (14.5%). CONCLUSION: The burden of cardiovascular disease in patients with psoriasis is huge. There was no statistical significance between the disease severity, duration of the disease and the risk factors in our study.
