Abstract

INTRODUCTION: HIV infection is associated with multiple endocrine organs involvement mostly adrenal and thyroid.. A high occurrence of abnormalities in thyroid function tests among HIV seropositive adults has been noted in previous studies. In addition, this thyroid dysfunction is correlated with advancement of the infection in conjunction with lowering CD4 cell counts. Subtle alterations in thyroid function tests are more common in HIV infection and at times detectable in the early phase of disease and as well as in late phases. However there is paucity of indian studies that are needed to evaluate the thyroid dysfunction in HIV infected patients and their clinical correlation. Hence this study is undertaken to study the thyroid dysfunction in HIV seropositive patients and correlate with the clinical features. OBJECTIVE OF STUDY: 1. To study the thyroid dysfunction in HIV seropositive patients and correlate with clinical features over the duration of 2 years. 2. To correlate the Thyroid function changes in these patients with their CD4 cell count. MATERIALS AND METHOD: In-patients and out-patients above 12 years undergoing treatment in MEDICINE department and ART centre at Tirunelveli Medical College and Hospital who are seropositive were selected with exclusion of known thyroid patients, diabetics and with those with abnormal liver and renal function tests. They were interviewed and thyroid function tests were carried out. RESULTS: In our study about 50 patients were studied who are seropositive and on ART. Out of them 12 Patients had TSH levels above 5uIU/ L. As age progresses CD4 count also deteriorates. The TSH level increases as CD4 count decreases. This implies as patient progressively deteriorate in AIDS, thyroid function also declines. CONCLUSION: In patients on ART thyroid functions tests should be routinely carried out for screening and patients should be followed subsequently both clinically and biochemically regarding thyroid replacement therapy.
