Abstract

AIM OF THE STUDY: In this prospective randomized control study to compare the three supraglottic airway devices named I-GEL, PROSEAL LMA, CLASSIC LMA in adult paralysed patients SETTINGS AND DESIGN: A prospective, randomized study was conducted in our Government Rajaji Hospital, Madurai. METHODS: The patients were randomly assigned to three groups I GEL (GROUP I), Proseal LMA (GROUP P) and Classic LMA (GROUP C). 30 patients were in each group). We assessed ease of insertion, attempts & time of insertion, hemodynamic changes, oropharyngeal sealing pressure, attempts of gastric tube passage and also postoperative complications. RESULTS: We have found that there were no statistically significant differences in the demographic and hemodynamic data among the three groups. The airway leak pressure of the Proseal group (22.06 ± 1.17 cmH20) was significantly higher when compare to that of the I-GEL group (21.9 ± 2.17 cmH2O) and the Classic LMA group (20.13 ± 1.24cmH20). The first attempt success rate (I GEL group) 27 of 30 patients and comparable to 26 of 30 in the PLMA group and 25 of 30 in the classic LMA group. Our study showed that the success rate for inserting the device was 90% on the first attempt and 100% after two attempts in I GEL group. The complications like airway trauma and sore throat incidence were very low in all cases, except for blood staining device in a few patients of P LMA group and C LMA groups, which was neither clinically & statistically not significant. Easy passage of gastric tube through P LMAs 28 patients (12 Fr) easily when compare to I GEL which was 10 patients.in first attempt with statistical significant. Insertion at second in PLMA group only in 2 patients and it was 18 in I GEL group. Insertion done at third attempt was 2 in I GEL group and nil in Proseal LMA group. CONCLUSIONS: We concluded that all the three supra glottic airway devices (I gel, Proseal, Classic LMA) can be safely used during GA in spontaneous and also in positive pressure ventilation. Comparatively P LMA had higher sealing pressure than I — gel and classic LMA. In terms of ease of insertion, attempts & time taken to insert, hemodynamic changes during peri operative period there was no significant difference between the three devices. Both I-GEL, P LMA have an added advantage of gastric channel comparing these 2 devices easy passage of gastric tube through P LMA was easier than I GEL group.
