Abstract

BACKGROUND: Squamous cell carcinoma is the common tumor of upper aerodigestive tract (UADT). Human Papilloma Virus (HPV) is a newly diagnosed risk factor. This study aims to analyze the distribution of squamous cell carcinoma of UADT in relation to age, gender, site, histopathological classification based on differentiation and their HPV correlation. Immunohistochemistry using p16 marker is a rapid tool to identify HPV related SCC. This study aims to determine the correlation between the histologic type and p16 marker. METHODS: The study was carried out in the department of Pathology, Madurai Medical College, Madurai, during the period from July 2016 to July 2018. After adequate fixation, bits were processed and stained with Haematoxylin and Eosin. The cases were classified based on WHO classification. In overall 496 cases of SCC, p16 IHC was done in 100 cases. Fischers exact test was used to determine the strength of correlation between the histologic type and p16 marker The observations were compared with other studies and inferences drawn. RESULTS: Upper aerodigestive tract (UADT) tumors constitutes 23% of total tumors received in our institute. Among them benign tumors were 32% and malignant tumors were 68%. In malignant tumors, SCC constitutes 94%. They were more common in males (74%) than females (26%) and conventional SCC were more frequently seen in 5th decade and HPV related SCC were seen during 4th decade. Among the histological types, conventional SCC is the most common type (65.5%) and rare variants were sarcomatoid (2%) and acantholytic type(1%). p16 was seen more in non keratinizing variant (59%) of SCC than the keratinizing SCC (3%). The sites of predilection for HPV related SCC were oropharynx and hypopharynx. By fischers exact test the p value was 0.001 and this proves a strong correlation between the histological type and p16 marker. CONCLUSION: There is positive correlation between the histologic type and p16 marker in SCC of UADT. Since HPV is a an independent prognostic factor. p16 should be used for identifying these SCC for planning further treatment modalities.
