Abstract

BACKGROUND: People from all walks of life are preys to cervical spondylosis. It is not uncommon that persons even in the fourth and fifth decades of life are suffering from cervical spondylosis. Fishermen, Rubber tappers, masons, drivers, teachers, IT professionals etc. are frequently visiting our hospital and are benefitted by the homoeopathic medicines. Even the students are also suffering from this neck pains at an early age. So I preferred to study on this area. A study about this disease by repertory will help to manage patients more effectively. A proper management depends on the selection of the similimum. Repertories are guiding the physician to the similimum. H.A. Roberts, felt the necessity of a repertory to the rheumatic remedies and he attempted to arrange the known rheumatic remedies under this great work. He emphasizes the great truth that the Hahnemannian disciple learns as a first principle of prescribing that the differentiation of the remedies lies in the differentiation of modalities, sensations and locations. There are chapters on modalities, symptoms in general (concomitants - mental generals and physical generals) and parts. Modalities and sensations are also there under all the chapters on parts. Chapters on neck, joints and upper extremities are very useful in the medicine selection for cervical spondylosis. . Trying to keep homoeopathic approach, he is giving a valuable aid in the selection of the similimum by this work and so I become interested in working out through his repertory. Kent’s repertory is a compilation of all the useful symptoms recorded in the fundamental works of material medica and as a constantly used repertory I wished to compare H.A. Roberts’ repertory with it. AIM OF THE STUDY: To study whether there is any significant difference in the effectiveness of simillimum selected using Repertory to the Rheumatic remedies by H.A. Roberts and The Repertory of the Homoeopathic Materia medica by Kent through clinical study in two different groups of patients suffering from cervical spondylosis by assessing improvement in disease with assessment by pre-test and post test scores. OBJECTIVES: 1. To study whether there is any significant difference after Homoeopathic treatment in cervical spondylosis by simillimum selected using Repertory to the Rheumatic remedies by H.A. Roberts 2. To study whether there is any significant difference after Homoeopathic treatment in cervical spondylosis by simillimum selected using Kent’s Repertory of Homoeopathic Materia medica. 3. To study whether there is any significant difference in the effectiveness of simillimum selected using Repertory to the Rheumatic remedies by H.A. Roberts and The Repertory of the Homoeopathic Materia medica by Kent by comparing their role in two different groups of patients suffering from cervical spondylosis. METHODS: Two samples each consisted of 30 cases presenting with the symptoms of cervical spondylosis were selected randomly from the cases visited the OPD, IPD and Rural centres of Sarada Krishna Homoeopathic Medical College. A scoring chart based on Neck Disability Index was formulated to assess the efficacy of treatment after the baseline assessment. Sample 1 consisted of 30 cases where medicinal selection was done with repertorisation using rubrics selected from H.A.Roberts’ Repertory to the Rheumatic Remedies and Sample 2 consisted of 30 cases where medicinal selection was done with repertorisation using rubrics selected from Kent’s Repertory of the Homoeopathic Materia medica. Follow up assessment was done once in 15 days for 2 months. The repertories were compared clinically by assessing the improvement after treatment with the help of Neck Disability Index. Paired “t” test was used to determine the variation of scores before and after treatment in each sample and “t” test was used for comparison to evaluate if the difference of repertories which was assessed clinically was statistically significant. RESULTS: The study was carried out in 60 patients affected by cervical spondylosis. Cases were followed up to 2 months. For remedy selection H.A Roberts Repertory was used in 30 cases where improvement in Neck Disability was marked and also in the other 30 cases where Kent’s Repertory was used as a basis for remedy selection, there was marked improvement in neck disability. Before and after test results were statistically verified. CONCLUSION: The medicines selected based on both H.A. Roberts’ Repertory and Kent’s Repertory were effective in managing cervical spondylosis and clinically there was no significant difference between the repertories. A tool will be familiar when it is frequently used. H.A. Roberts Repertory as it is not frequently used and the construction is not user friendly the search for rubric frequently reminded the need for studying the materia medica part before using the author’s hasty reminder, the repertory. But Kent’s repertory can surely claim as a user friendly tool which was born out of logic.
