Abstract

INTRODUCTION: In India, Type 2 Diabetes Mellitus is in the potential epidemic state and one of the major cause of chronic kidney disease. Early detection of renal dysfunction helps to prevent further progression of renal damage. AIM OF THE STUDY: To evaluate the usefulness of “Protein Tolerance Test” as a method of measuring “Renal Functional Reserve” in Type 2 Diabetes Mellitus and to compare with normal subjects. MATERIALS AND METHODS: A total of 104 subjects were subjected to a Protein Tolerance Test using 100 gm of protein as cottage cheese. Serum creatinine levels at 0, 60 &120 minutes and Urine Protein creatine ratio at 0 &120 minutes were obtained. Renal Functional Reserve measured by e GFR using Cockcroft- Gault Equation. The results were analyzed statistically. RESULTS AND OBSERVATIONS: In our study it was found that, Renal Functional Reserve reduced in persons with Type 2 Diabetes Mellitus than controls. Among the cases, persons with older age, longer duration of diabetes, poor metabolic control had more renal dysfunction. The mean e GFR values at 0 min were 71.95 and 76.83 ml/min/1.73 m2 in cases and controls respectively. The mean e GFR values after Protein meal were 81.28 and 95.38 ml/min/1.73 m2 in cases and controls respectively. The prevalence of Renal dysfunction was more in the cases (40.4%) compared to controls (3.8%). All the results are statistically significant (p=0.0001). CONCLUSION: Protein Tolerance Test can be very useful to assess the Renal Functional Reserve and detect incipient renal failure in a person with normal GFR and serum creatinine values, thus identifying patients who are most likely to be benefited by an aggressive intervention.
