Abstract

The Research undertaken is “An experimental study to evaluate the effectiveness of need based intervention on selected psycho social parameter among children with nocturnal enuresis”. The abstract for the same is presented in (IMRAD) Introduction, Method, Results and Discussion format for writing. INTRODUCTION: Nocturnal Enuresis is a very common group of disorder in childhood, which occurs world-wide. It occurs in 10% of all 6 year old children and is more common in boys than girls. It is estimated that every morning about five to seven million children wake up in a wet bed, due to primary nocturnal enuresis. Differences were between the ethnic groups comprised 75.1% were in Malaysia, 11.7% were in Indians and 11.4% were in Chinese. The prevalence of both PNE and SNE was highest among Indians and lowest among Chinese children (Source DGM 2012). World Bedwetting day (WBD) marked globally on May 24th, brought awareness among public and health care Professionals on Nocturnal enuresis. The impact on the children’s QOL and stress were also focused. Behavioral interventions and exercises are helpful in overcoming this problem. Techniques recommended by behavioural health practitioners and physicians includes pediatric pelvic girdle exercises, that improves pelvis stability by contracting lower abdominal muscles, which is good for bladder control. This will help the children to become more aware of their bladder sensations, such as feeling of fullness and urge to urinate. Breathing exercises are also used as stress reduction technique, which links the body and mind and gives natural relaxation response to the body. Breathing exercises are one of the best ways to lower the stress in body because when children breathe deeply, it sends messages to brain to calm down and relax. German Society for children and adolescent psychiatry recommended specific therapy baseline which involves parental education, positive reinforcement, counseling, diet and fluid intake recommendations, lifting and waking methods, and motivation with sun-cloud calendars. OBJECTIVES: 1. To assess and compare the pre and post test score of psycho social parameter among children with nocturnal enuresis in experimental and control groups. 2. To evaluate the effectiveness of need based intervention on selected psycho social parameter among children with nocturnal enuresis. 3. To correlate the relationship between the psychological and social parameter among children with nocturnal enuresis in experimental and control groups. 4. To associate the selected demographic variables and psycho social parameter of children with nocturnal enuresis in experimental and control groups. The research process for the study was guided by the conceptual framework based on Ludwig Von Bertalanffy General System Theory (GST) and J.W. Kenny’s Open System Model. RESEARCH METHODOLOGY: Quantitative research approach and quasi experimental research design which includes control group and manipulation with no randomization was adopted for the study. The Independent variable was need based intervention (psycho education, structured counseling and exercises) and dependent variable was psycho social parameter (Stress and QOL). The setting of the study was Child Guidance Clinic, Government Institute of Child Health, Egmore, Chennai. Non probability purposive sampling technique was used for selection of samples. Total enumeration of all nocturnal enuresis children were identified as clinically significant by using stress and QOL rating scale. Sample size comprised of a total estimation of 300 children. Stress and QOL were assessed through 3 domains like physical, emotional and social. The nocturnal enuresis children were categorized as having mild, moderate and severe level of stress and moderate, low and poor level of QOL, after which need based intervention were administered for the period of 3 months. Need based intervention consists of psycho education by using flash cards and booklets, structured Individual counseling followed by demonstration of pediatric pelvic girdle and pediatric calm breathing exercises. The tools were validated by various experts. The pilot study revealed that tools were reliable and study was researchable. The investigator considered and followed ethical principles preceding the investigation and also adhered to human rights, beneficence and non maleficience, dignity, confidentiality and justice. RESULTS: • SPSS statistical package was utilized for the data analysis. • In pre test, mean score for psychological parameter of stress in experimental and control groups were 46.17 and 48.87 respectively while in post test, the mean score was 29.63 and 44.91 respectively. The mean difference was 16.54 in experimental group and 1.96 in control group. The calculated ‘t’ value in experimental group was 18.75, which showed significant at the level of p<0.001, but the calculated ‘t’ value in control group was 1.92, which was not statistically significant. • In pre test, mean score for social parameter of QOL in experimental and control groups were 47.42 and 46.35 respectively. In post test the mean score was 30.10 and 44.09 respectively. The mean difference was 17.32 in experimental group and 2.27 in control group. The calculated ‘t’ value in experimental group was 18.75, which showed significant at the level of p<0.001, but the calculated ‘t’ value in control group was 1.89, which was not statistically significant. • The psychological parameter of stress, considering pre and post test, among children, in experimental group were with 20.7% stress reduction, while in control group stress reduction was 2.5%. The difference between pre and post test in both groups were 18.5%-22.8% in experimental group and 1.5%-3.4% in control group. The calculated ‘t’ value in experimental group was 14.80, which showed significant at the level of p<0.001, but the calculated ‘t’ value in control group was 1.17, which was not statistically significant. • The social parameter of QOL, considering pre and post test in experimental group children were with 21.7% QOL improvement, while in control group only 2.8% QOL improvement was noted. The difference between pre and post test in both groups were 19.7% to 23.6% in experimental group and 1.9% to 3.8% in control group. The calculated ‘t’ value in experimental group was 15.79, which showed significant at the level of p<0.001. While the calculated ‘t’ value in control group was 1.53, which was not statistically significant. • There was a significant positive moderate correlation between stress and QOL in experimental group with ‘r’ value of 0.56, which was significant at the level of P≤0.001. But in control group the ‘r’ value was 0.20, which was not statistically significant. • There was a significant association between the selected demographic variables with psycho social parameter of children with nocturnal enuresis in experimental and control group. CONCLUSION: The findings of the study showed that distribution of Nocturnal enuresis among children in both groups were homogeneous. This study also indicates that the need based intervention administered to the children with nocturnal enuresis were effective in reducing the psychological parameter (Stress) and improving the social parameter (QOL) in experimental group than children in control group.
