Abstract

INTRODUCTION: Cerebrovascular disease is one of the most common and devastating disorder. Stroke is the second most common cause of death in the world. It is one of the most common life threatening and disabling neurologic disease. Multiple studies demonstrated the role of CRP as a predictive marker for cerebrovascular events. Many studies evaluated only the relationship between CRP and mortality instead of functional outcome.Insearch for further clarification we sought to assess CRP level as an early prognostic marker of functional outcome after stroke. AIMS AND OBJECTIVES: To evaluate the role of CRP assay within 24 hours of stroke onset as a biomarker for predicting disease severity and short term outcome (within 1 week). MATERIALS& METHODS: This study was conducted in Mahatma Gandhi Memorial Govt. Hospital attached with KAPV Govt. Medical College Trichy from January 2018 to January 2019.In this 50 patients who were diagnosed to have first ever acute stroke who presented within 24 hours of stroke onset were enrolled. CT scan was done to differentiate between ischemic and haemorrhagic stroke. Three ml venous blood samples were taken for CRP levels. Stroke severity at admission was assessed using NIHSS. Stroke outcome after seven days of stroke onset was assessed using Modified Rankin scale(mRS) and Barthel ADL index(BI). Using statistical analysis correlation between (NIHSS, CRP), (mRS, CRP), and (BI, CRP) were assessed separately. RESULTS: CRP levels were not correlating significantly with NIHSS, mRS, BI levels in the total study group of stroke patients. CRP levels were positively correlated with NIHSS and the results were statistically significant in ischemic stroke patients. CRP levels were negatively correlated with BI levels and results were statistically significant in ischemic stroke patients CRP levels were inappropriately correlating with all the three scores in haemorrhagic stroke patients. CONCLUSION: So, final conclusion is that CRP levels at admission can predict the severity of stroke in ischemic stroke patients. It can also predict the outcome using BI in ischemic stroke patients.In haemorhhagic stroke patients CRP levels are not useful for predicting stroke severity and outcome.
