Abstract
INTRODUCTION: Under this topic of this dissertation, the author wants to explain the pathological view of the disease ‘SAYA PITHAM’ which helps to understand the Etio Pathogenesis, Pathology and Naadi – Nadai of the disease and to treat the ailment properly. Saya Pitham is related to the disease of the ‘PRAANAMAYA KOSAM’ (Respiratory System). The humors which involves mainly in the origin and prognosis of the disease are PITHAM and KAPHAM. The three uyirthaadhukal (cardinal humors), Vali, Azhal and Iyam plays a vital role in maintaining the body and mind in a healthy condition. These three uyirthaadhukal should be in normal, increased on varied (தன்னிலை, தன்னிலை வளர்ச்சி, வேற்று நிலை வளர்ச்சி) as per the seasonal changes (PERUM POZHUTHUGAL). If any abrupt changes occurs in this chain, the disease occurs. AIM OF THE STUDY: To study the disease on the basis of siddha physiology, siddha pathology by the panchabootha Theory, Uyir Thathukkal, Udal Thaadhaukkal and diagnose the patient on the basis of Ennvagai Thervugal, Manikkadai and confirm the prognosis of the disease through Neikkuri. OBJECTIVES: 1. To collect all literary evidences about the disease in detail. 2. To study each and every aspect of the disease “SAYA PITHAM” regarding its aetiology, humoral Pathogenesis, signs and symptoms, fate of disease in Siddha aspect. 3. To study in detail about the incidence of the disease with age, sex, occupation, socioeconomic status, personal habits and thinai. 4. To know the extent correlation of aetiology, signs and symptoms of SAYA PITHAM with that of modern aspect. 5. To bring forth the highlights of diagnostic methods by the siddhar’s like ennvagai thervugal and manikkadai nool. 6. To utilize the modern parameters for the confirmation of diagnosis. MATERIALS AND METHODS: Study Design: Observational type of study. Study Place: The study of the disease saya pitham in Noi Naadal aspect was carried out at the outpatient department of PG- Noi Naadal, Govt. Siddha Medical College and Hospital, Palayamkottai. Study Subjects: Case selection And supervision. The author had selected 29 cases with similar symptoms of SAYA PITHAM as mentioned in “Pitha Rogha Nithanam” Yugi Vaithiya Sinthamani, under the supervision of faculties and head of the department of PG Noi-Naadal Department. Out of 29, 20 cases are selected and remaining 9 are excluded for varied reasons mentioned under the exclusion criteria. These 20 cases were selected to evaluate the typical picture by using siddha parameters and allied parameters. The detailed history of present and past illness, personal and family history, etc., were observed. All the 20 cases were selected to evaluate the typical picture by using siddha parameters and allied parameters. PREVENTIVE MEASURES: The following measures are to be observed to prevent the disease saya pitham. ❖ Smoking cessation can slow the progression of the disorder. Avoiding secondhand smoke and other lung irritants is also recommended. ❖ Moderate aerobic activity may help prevent progession of the disease and will generally improve the capacity for exertion. ❖ Taking measures to prevent chest infections, which include frequent hand washing and vaccinations, is advisable. ❖ Drinking plenty of fluids and breathing moist air (for example, by using a humidifier) help thin mucus secretions. In contrast, cold, dry air should be avoided. ❖ Always wear protective clothes and mask when working in a environment. ❖ Be extra cautions during seasonal changes as frequency of attacks increase in this duration. ❖ Protect mouth and nose when going out in cold climates so that the person does not breath in cold air as cold air is known to make the condition worse. ❖ The patients should regularly go for walk in clean and healthy like environment park and take deep breath and stay stress free. ❖ Daily yogic exercises are remarkably beneficial for patients. ❖ Some of the yogic breathing exercises are effective. ❖ Praanayamam and other breathing exercise are effective as they improve blood circulation and cure the breathing difficulties. CONCLUSION: SAYA PITHAM has been described in PITHA ROGHA NITHANAM in YUGI VAITHIYA SINTHAMANI. The clinical study of SAYA PITHAM is mainly based on the siddha diagnostic methods. At the same time the author had also examined with modern parameters relevant to SAYA PITHAM. It concludes that all SAYA PITHAM patient most probably had the features of chronic bronchitis. The siddha diagnostic parameters such as Ennvagai thervugal, Manikkadai Nool, Neikkuri give a clear to diagnose the disease ‘SAYA PITHAM’. The clinical features of ‘SAYA PITHAM’ are cough, Sputum production which may be blood streaked, mild fever, burning sensation in chest, paleness, excessive sleep. In modern aspect, the symptoms of SAYA PITHAM may be correlated with the clinical features of CHRONIC BRONCHITIS
