Abstract
OBJECTIVES: 1. Study difference in incidence of adverse mother & neonatal outcomes in pregnant women with hypothyroidism vs those without. 2. Determine screening of thyroid disorder be offered to all pregnant women. METHOD: A Prospective cohort study was done comparing the obstetrical and neonatal outcomes of pregnant women with hypothyroid and those without. The pregnant women with hypothyroidism and newly diagnosed were recruited to Group 1. Their thyroid medication dose was titrated as per TSH. Pregnant woman with normal thyroid function were enrolled into Group 2. The pregnant women of both groups were followed till delivery and postpartum. The primary and secondary outcomes of both mother and child were analysed. RESULTS: The study showed that evolution of disease during pregnancy does not depend upon the degree of hypothyroidism, whether overt or subclinical, but on the oral thyroid therapy received. Timely initiation of therapy and close treatment monitoring to titrate the dose of individual thyroid medications is of paramount importance in prevention of obstetric and neonatal complications. CONCLUSIONS: In conclusion, we found that evolution of disease during pregnancy does not depend upon the degree of hypothyroidism, whether overt or subclinical, but on the therapy received. Timely initiation of therapy and close treatment monitoring to titrate the dose of individual thyroid medications is of paramount importance in prevention of obstetric and neonatal complications.
