Abstract

AIM AND OBJECTIVES: Even though our country is developing one, some of the people are still below the poverty line, under poor economic status. They have been suffering from various diseases due to deficiency of nutritents. It is also stated that about one-third of the total world population and 60-70% of the Indian children are affected by Pandu noi. It is essential to findout a simple drug to treat this disease. The drug should be easily available, economic, easily administered and also effective in smaller doses. I found from our siddha literature that “THIRIPALA MATHERAI” possess the above characters. This is the reason for selecting this drug. The aim of the study on “PANDU NOI” is to create a new approach in diagnosis for this thesis and to find out a safe, perfect remedy. OBJECTIVES: 1. To take authentic measures and review the ideas of “PANDU NOI” as indicated in Siddha Literature and to know the efficacy of the trial medicine. 2. To have an idea about the prevalence of Pandu Noi with reference to Age, Sex, Socio-economic status, Seasonal variations, Poverty, etc., 3. To know the extend of correlation of aetiology, classification, symptomatology, diagnositc methods and line of treatment compared with iron deficiency anaemia. 4. To know the alteration of the disease under the topics of Mukkutram, Udal Kattugal, Poripulangal, Envagai Thervugal, Neerkuri, Neikuri. 5. To make a clinical trial of patients with the trial medicine “THIRIPALA MATHERAI” in the treatment of Pandu Noi. 6. To make use of modern parameters in the investigation side to confirm the diagnosis and to follow the progress of patients. 7. To elicit the biochemical analysis and pharmacological action of the trial medicine. 8. To make an awareness among the parents about preventing the disease in children. MATERIALS AND METHODS: The study on the clinical evaluation of the disease Pandu noi was carried out in the Postgraduate Kuzhanthai Maruthuvam Department in Government Siddha Medical College, Palayamkottai. Thirty patients of both male and female children were selected for the study and admitted in the Postgraduate, Kuzhanthai Maruthuvam In-patients department. After discharge all of them were advised to come to the out-patients department for further follow-up. Another 40 patients were also treated with trial drug in the Out-patients Department. Selection of patients: The present study covers both male and female children of varying age groups. All cases were carefully and thoroughly examined before admission. Those who fulfilled the criteria for Pandu noi (Iron deficiency anaemia) according to the patho-physiology of Siddha and Modern reviews were selected. The opinion of professor and lecturer was obtained and detailed history was recorded in the proforma of case sheet. INCLUSION CRITERIA: • Age : 1- 12 years. • Sex : both male and female children. • Children with Iron deficiency Anemia with Hemoglobin less than 10 gms/dl. • Families with more number of children. • Families with poor economic status. • Children having the symptoms of Iron deficiency anemia. Pallor of skin and mucus membrane. • Decreased or loss of appetite. • Fatigue. • Having perverse taste to eat mud, sand, ash etc or increased food cravings (pica). • General weakness. • Irritability. • Brittle nails. • Shortness of breath. • Palpitation. • Tachycardia. • Angular stomatitis. • Patients willing to give blood and urine sample for lab investigations whenever required. • Willing to give blood and urine sample for lab investigations whenever required. • Informed written consent given by parents/guardian of the patients. EXCLUSION CRITERIA : • Anemia due to causes other than Iron deficiency anemia. • Bleeding disorders. • Severe concurrent illness like (cardiovascular, renal & hepatic). • Malignancy of any type. • Thalassemia major. • Aplastic anemia. • Sickle cell anemia. • Haemolytic anemia. • Rheumatic fever. • Nephrotic syndrome. • Tuberculosis. SUMMARY: The study of Pandu noi is done to find out a complete relief to those affected, with a herbal and mineral combination of simple preparation Thiripala matherai. Various literature evidence revalent to Pandu noi were colllected from both Siddha system as well as modern system of medicine. The effficacy of the drug the Thiripala matherai has been studied and observed. Twenty patients from both sexes of different age groups were selected and treated in the In-patient ward of PG- Kuzhanthai Maruthuvam Department of PG Kuzhanthai Maruthuvam Deprtment of GSMC. Among the 20 pateints, 7 were male children and 13 were female children. Further follow up of the cases were done in the outpatient ward. Specific investigations and the prognosis of the patient were studied and the proforma was prepared accordingly, Maximum occurrence of this disease was observed in school going children (6 to 12 years) but the other have no excemption. Findings reveal about the impact of the disease in the body. Statistical study of the details in the case sheet was observed and the results have a see through idea about the disease. The drug selected for the study was found to be easy for administration and the children found it easy for intake. No adverse side effects were reported during or after the course of treatment. The biochemical analysis and pharmacological studies of the drug revealed its efficacy. From the studies, the drug Thiripala matherai with Sombu Theeneer possesses significant haemantinic action. CONCLUSION: From the above studies it is clear that Pandu noi, is caused by derangement of pitham, followed by the derangement of vatham and kabam. The drug taken for the study THIRIPALA MATHERAI has been proved clinically to be potent haematinic and it raises the hemoglobin level in anemia patients when given regularly along with other healthy supplementary diets. Both symptomatic relief and qualitative improvement were observed in the patients. These drug is easily available and cost effective. Due to encouraging result of above study these drug may be taken for larger study in treatment for Pandu noi and it will open new windows.
