Abstract

INTRODUCTION: The tibia is the most commonly fractured long bone. Owing to its location and the tenuous soft tissue coverage, open fractures are more common in the tibia than in any other bone . Open tibial fractures are showing an increasing trend due to road traffic accidents and firearm injuries. The situation is all the more alarming in developing countries such as India where the industrial safety norms are ignored and the Traffic management is poor. AIMS AND OBJECTIVES: 1. To Evaluate 25 cases of open tibial fracture which have been treated with Ilizarov Ring Fixator as per fracture pattern. 2. To Analyse the functional outcome following definitive fixation of these injuries using Tucker’s criteria. MATERIALS AND METHODS: Place government royapettahhospital,Kilpauk medical college,chennai Total number of cases -25. Inclusion criteria : • Patients in the age group of above 18 years. • Both male and female patients. • Patients with compound fractures of tibia. • Associated with neuro vascular injuries. • All grade of gustiloanderson classification (1 to 3c). • Compound fractures of tibia < 7days. Exclusion criteria: • Age less than 18 years. • Pathological fractures. • Compound fracture >7 days. • Crush injuries. • Associated with Compartment syndrome. RESULTS AND CONCLUSIONS: The Ilizarov external fixator is a versatile instrument in the armentarium of an orthopaedic surgeon. We conclude from our study that though ilizarov surgery is effective tool for treating open tibia fractures and it may prove to be equally effective in dealing with the soft tissue aspect of these injuries. Utilizing compression at the fracture site will not only promote bone healing but may allow closure of wounds that previously required flap coverage or amputation. Though technically demanding surgery with minor treatable complications, it has higher rate of union than any other methods of surgery in this type of fracture. we recommend IEF as the definitive primary fixation in type II, IIIA and IIIB fractures.
