Abstract

To find the root cause of the illness, the Siddha system of medicine, recommends eight powerful tools called “Ennvagai thervugal”. In this study, the author opted to elaborate the examination of Tongue based on Siddha Principles. In ancient literatures by Sage Theraiyar, Sage Agathiyar features of tongue in various diseases are depicted. These tools not only help in the diagnosis but also to assess the prognosis and restoration of health in the diseased individual. As specified by the Sage Yugi, in his treatise has mentioned various characteristic features of tongue according to tridhoda thegi concepts. ❖ Vatha thegi’s tongue appears to be black in colour and burst like thorn, Pitha thegi’s tongue appears to be reddish in colour, Kabha thegi’s tongue appears to be whitish in colour and with increased saliva, Thontha thegi’s tongue appears to be multi-coloured. Naadisasthiram also explains the thridhoda concept of tongue diagnosis. Sage Agathiyar in his treatise has mentioned various characteristic features of tongue according to tridhoda concepts. ❖ The clinical protocol of this study had been approved by the Institutional Ethics Committee of NIS, Chennai, India vide approval Ref. No. NIS/IEC/2020/D 28;21/12/2020 and it had been registered with Clinical Trial Registry of India with Regn.No. CTRI/2021/08/035972. ❖ In National Institute of Siddha hospital 385 patients were screened and the 360 number of patients fount eligible were enrolled. Tri humoral tongue character is described in Noinaadal noimuthal Naadal part-I [2]. Regarding this gender, it is found that 183 were male and 177 were female patients (ref. Table 1 Gender Distribution). Among 360 cases, 30 (8.33%) patient was pure vegetarian, 330 cases that is 91.66% of them follow mixed diet (ref. Table 2 Diet History Among the subjects). The Sample Registration Baseline Survey[55] reveals that as of January 2014, 71 percent of Indians were non-vegetarians. This is due to the greater number of patients belongs to the category of non-vegetarian in Tamilnadu. In this study 91.66% of subjects were non-vegetarians and 8.33% of subjects were vegetarians. ❖ While observing the patients’ personal habits (ref. Table 3 Distribution of habits), 54 (15%) cases had history of smoking, 23 (6.38%) cases had history of chronic alcoholism, and 240(66.66%) a cases had a habit of drinking tea and 27(7.50%) a cases had a habit of drinking coffee. Frequent intake of tea /caffeinated drinks may lead rheumatoid arthritis and inflammatory reaction in joints. [56] Likewise in this also people who were regularly taking tea/coffee presented with the complaints of diseases pertaining to bone and joints ❖ Though moderate intake is healthy for most people, drinking too much could lead to negative side effects, such as anxiety, headaches, digestive issues [57] and disrupted sleep patterns that lead to increasing vatha disease. ❖ Regarding the vatha naa character (ref. Table 4 Vatha Naa characters) 12.22% cases had karuthiruthal, 36.11% and 39.72% male and female cases had pilappu character 40.55%, 7.50% male and female cases had varatchi character. In this study the cases were included as per the naa characters given in Yugi Vaithiya Chinthamanai – 800[4]. ❖ Regarding the kaba naa character (ref. Table 6 Kabam Naa charactersTable 4 Vatha Naa characters) 19.16% and 46.94% male and female cases had maapadithal. In this study the cases were included as per the naa characters given in Yugi Vaithiya Chinthamanai – 800. ❖ In this study majority in naadi and naa matching (ref. Table 8 matching of Naa characters with Naadi, Migu Gunam, Yakkai Ilakkanam respectively) 16.17% and migugunam and naa matching 21.45% 11.14% yaakai and naa matching was very less. ❖ In this study majority in 1st humour of naadi and naa matching (ref. Table 9 Matching of 1st Humour of Naa with Naadi, Migu Gunam and Yakkai Ilakkanam) 28.13% and migugunam and naa matching 55.71%. 30.36% yaakai and naa matching was very less. In this study naa character matching with op diagnosis 52.92% case had matched. ❖ The prevalence (ref. Table 11 Common tongue lesions and associated symptoms among the subjects) of common tongue character and conditions and associated symptoms among the study subjects were analysed With Chi square test, p-value (0.000) and found that the eight tongue parameters are associated with each other. ❖ The Naa kuttraverupadu was analysed with Naadi and migugunam with Fisher Exact test - Monte Carlo method (ref. Table 13 Fisher Exact test of observations) and found that the four parameters are associated with each other. ❖ So in this study it was found out that there is statistically and clinically significant association between naa kurta verupadu and naadi, as well as naa kurta verupadu and migu gunam of each components of the three humours. CONCLUSION: In this study of Tri-humoral categorization through Naa (Tongue) examination, it is revealed that there is statistically and clinically significant association between Naa kurta verupadu (Trihumoral features of tongue) and Naadi (Pulse play), as well as between naa kurta verupadu and migu gunam (features of excessiveness) of each components of the three humours. So this study affirms the reliability of tri humoral tongue character along with Naadi as stable diagnostic tool of tri-humoral nature of the body. So, this study reveals that tongue is one of the diagnostic tools that parallels the Naadi diagnostic tool in Siddha system of medicine.
