Abstract

The study aimed to evaluate the therapeutic efficacy of the Oma Ilagam (internal) and Sangamver Thylam (External) for the Management of Kalanjagapadai (Psoriasis) in Children. ❖ Before initiating the clinical trial, approval has got from the Institutional Ethical Committee on the National Institute of 21.12.2020: (NIS/IEC/2020/D-22) for conducting the clinical study. ❖ The trial was registered prospectively in the Clinical Trial Registry of India and the number: CTRI/2021/08/035820 (Registered on date 19/08/2021). ❖ The raw drugs were authenticated by the Assistant professor of medicinal botany, NIS. ❖ The trial drug was prepared in the Gunapadam lab of the National Institute of Siddha as per the standard operating procedure in the protocol. ❖ Physicochemical, phytochemical analysis and pharmacological activity were done in Noble research solution, Chennai. ❖ Loss on Drying at 105 °C -20.57 ± 1.96, Total Ash 0.70 ± 0.10, Acid insoluble Ash 0.056 ± 0.006, Water soluble Extractive 7.4 ± 1.21, Alcohol Soluble Extractive 13.2 ± 2.00. Low acid insoluble ash indicates purity of the trial drug Oma Ilagam. ❖ The water-soluble extract of Oma Ilagam 7.4 ± 1.21 and alcohol soluble extract was 13.2 ± 2.00. water soluble extract indicates the good medicinal absorption and action. ❖ The trial drug Oma Ilagam was free from microbial contamination and specific pathogen. In heavy metal analysis lead and arsenic were present below the detection limit. The trial drug Oma Ilagam was free from aflatoxin and pesticide residues. ❖ In HPTLC analysis showed Rf value of the peaks ranges from 0.04 to 0.61. This sample revealed the presence of six versatile phytocomponents present within it. ❖ In phytochemical analysis of Oma Ilagam revealed that presence of alkaloids, flavonoids, steroids, coumarin, phenol, tannin, saponin, sugar and betacyanin. Saponin worked as anti-inflammatory, immunomodulatory and anti histamine and it helps to the itching, scaling and erythema. ❖ Biochemical analysis of Oma Ilagam showed the presence sulphate, carbonate, calcium, Iron, Magnesium and Zinc, starch,alkaloids and sugar. Which were present in this trial drug Oma Ilagam. ❖ Trial drug Oma Ilgam had revealed that Immunomodulatory activity in reducing nitrite level in raw macrophage cell line in vitro study. ❖ Trial drug Sangamver Thylam had revealed that anti- inflammatory activity and anti histamine in vitro study by albumin denaturation assay. ❖ Trial drug Oma Ilagam as internal medicine was given for 45 days at a dose of 1.3g for 5-7 years of children and 2.6g for 8-12 years of children, twice a day with Milk. Sangamver Thylam as external medicine was given for 45 days at a dose of required quantity based on the severity of area. ❖ The prognosis was assessed clinical assessment done on first day and the end of the 46th day. The prognosis has recorded through PASI Score and Quality of life has recorded through CDQLI score. ❖ The outcome has clinically observed by PASI score, the Children. 50% (15) were shown Good and 40% (12) were improvement moderate and 10% (3) were shown mild improvement. ❖ The outcome has clinically observed by CDQLI score, the Children, 60% have shown Good and 37% shown moderate improvement and 3% have shown mild improvement. ❖ Statistical analysis was done for PASI Score and CDQLI Score, before and after treatment has been Significantly reduced. (p 0.0001). ❖ None of them reported recurrence of disease during this study period. No one had reported adverse effects during the trial period. All these above analytical study ensure the safety and effectiveness of the trial drug. All these above conclude that the trial medicines were good medicine of choice for Kalanjagapadai and given good result. CONCLUSION: From this study, it is concluded that, the trial drugs Oma Ilagam as internal medicine and Sangamver Thylam as external application are significantly effective and safest formulations in the treatment of Kalanjagapadai (Psoriasis) in children. Since the trial medicines Oma Ilagam has immunomodulator activity and Sangamver Thylam has anti-inflammatory and anti-histaminic activity it may be the reason for the reduction in scaling, erythema, itching and thickness on skin. This study provides significant values from the PASI Score findings. Thus the finding shows good improvement in 50% (15), moderate improvement in 40% (12) and mild improvement in few children 10% (3). ❖ This study may be further carried out in a larger population to get better understanding about these drugs scientifically.
