Abstract

The clinical study on Azhal Neer Churukku was carried out in Post Graduate Department Of Pothu Maruthuvam, Government Siddha Medical College, Arignar Anna Hospital Of Indian Medicine Chennai-106, during the period of 2019-2022. ❖ A total of 40 patients were treated in the Out patient department. The clinical and pathological assessment was carried out on the basis of Siddha and Modern aspects. ❖ All the patients were treated with KANDHAGA RASAYANAM twice a day, after food for 15 days. ❖ Majority of the patients were in the age group of 51-60 years. ❖ Prevalence is more among female gender (52.5%). ❖ Prevalence is more among Home makers and daily wages workers. ❖ Majority of the patients were reported in the vaatha kaalam (82.5%). ❖ 52.5% of the cases were in the Middle Income People. ❖ 92.5% of the patient had mixed diet. ❖ Most of the patients were affected in Koothir kaalam (55.50%) ❖ Most of the patients were from Neithal thinai. ❖ All the patients have Raasatham gunam. ❖ In Vatham, Before treatment Abanan (100%), Vyanan(92.5%), Udhanan (25%), Samanan (90%) were affected . After treatment with study Medicine it is reduced to 7.5%, 7.5%, 7.5%, 7.5%. ❖ In Pitham, Before treatment, Analpitham (37.5%), Ranjagam (10%), Sathagam was affected in (37.5%), were affected. After treatment with study Medicine it is reduced to 7.5%, 5%, 10% respectively. ❖ In Kabham, Before treatment, Kilethagam (27.5%), Santhigam (10%) were affected. After treatment with study Medicine 87.5%, 97.5% of patients were recovered. ❖ In Udal Kattugal, Before treatment Saram (37.5%), Seener (10%), oon (35%) were affected. After treatment with study Medicine it is reduced to 10% and senneer became normal. ❖ In Envagai thervu, Regarding Naadi (40%) of patients had vatha pitham in before treatment and after treatment 72% of them had Vatha pitham. ❖ Regarding Naa in before treatment (20%), Niram (7.5%), Vizhi (10%), Malam (22.5%), Moothiram (100%) were affected. After treatment with study Medicine it is reduced to 5%, 2.5%, 2.5%, 5%, 5%. ❖ In Neikuri before treatment more number of patients are having pitha neer (30%) after treatment more number of patients are having vatha neer (55%). ❖ Regarding clinical prognosis, after treatment there is significant reduction in symptoms like burning micturition, frequent micturition, supra pubic pain, low back pain, urgency of micturition, incomplete voiding of urine.The p-value was 0.001. ❖ Regarding laboratory investigations, Dip stick test before treatment 92% of patients had Nitrite positive in their urine. After treatment nitrite became negative in 87.5%. and leucocyte esterase became negative in all the patients. In urine culture 62.5% of the patients had E.coli negative in their urine and klebsiella is negative after treatment in all the patients reported positive. ❖ The toxicological study reveal that the trial medicine possesses no toxicity in animals. So, it proved the safety of the trial medicine. ❖ Bio – statistical analysis of the clinical trial reveals significant p-value< 0.005 and <0.001 and concluded that treatment is effective and significant. ❖ The Outcome of the study showed that out of 40 cases, 10% of the patients having mild improvement, 12.5% are having moderate level improvement and 77.50% are having good level outcome. CONCLUSION: Azhal Neer Churukku (Urinary Tract Infection) occurs due to the derangement of Pitha kutram. The trial medicine, Kandhaga Rasayanam neutralizes the increased pitham. Most of the ingredients of Kandhaga Rasayanam has anti microbial activity and anti inflammatory activity, and hence it countervailed the deranged Uyir thaathu and provided ease to the patients with Azhal Neer churukku. The Kandhaga Rasayanam reveals no toxicity in the preclinical studies and hence proved to be safe for human administration. No contra indication was reported during the course of the treatment. From the clinical trial it is evident that the Kandhaga Rasayanam regulates the clinical symptoms and improvement in laboratory parameters especially patients with culture positive with E.coli and Klebsiella. The preparation of trial medicine is so easy and economical. Therefore, the author concluded that, the trial medicine KANDHAGA RASAYANAM can give better solutions for AZHAL NEER CHURUKKU (Urinary Tract Infection). FUTURE PERSPECTIVE: ❖ As the author had certain limitations in sample size and duration for conducting the study, the author decides to do in large sample size with long term duration in upcoming years. ❖ As the Medicine has no toxicity and safe in terms, chronic toxicity study has to be done in Kandhaga Rasayanam for Azhal Neer Churukku ❖ As the study Medicine is easily available, so it can be utilized for huge population in public health sector for benefit of the society.
