The study aimed to evaluate the therapeutic efficacy of the Vallarai Nei (internal) and Sembarithipoo ennai (External) for The Management of Kalanjagapadai (Psoriasis) in Children. • Before initiating the clinical trial, approval has gotten from the Institutional Ethical Committee of the National Institute of Siddha NIS/IEC/2019/D-29 & 13-09-2019 for conducting the clinical studies respectively by submitting the well-defined protocol proforma. • The trial was registered prospectively in the Clinical Trial Registry of India and the number: CTRI/2020/06/026154 (Registered on date 26/06/2020). • The raw drugs were authenticated by the Assistant professor of medicinal botany,NIS. • The trial drug was prepared in the Gunapadam lab of the National Institute of Siddha as per the standard operating procedure in the protocol. • As per Siddha Materia Medica, the trial drug's ingredients have found to possess anti-psoriatic, anti-inflammatory, immunomodulator and anti-pruritic activities that might have a significant role in treating Kalanjagapadai. • The Chemical, physicochemical, Phytochemical, HPTLC, heavy metal analysis, pesticide control, Aflatoxin of the trial drug have been carried out. • Out of 50 cases, 30 cases were recruited for the clinical trial. Both Siddha and modern methodology made a clinical diagnosis of Kalanjagapadai. Before initiating the clinical trial, informed consent was obtained from all the patients. • All the patients were advised to for oil bath with Arakku oil on Day -1 and Day-2 they were given purgation with Meganathakuligai100mg with Hot water, Day-3 patients were advised to take rest, no medicine was given on that day to correct the elevated mukkutram. • From Day-4 onwards,Patient have treated for 45 days with Vallarai Nei-5 ml (internal) and Sembarithipoo ennai-50 ml (External). • During each visit in OPD patients (7 days once), clinical assessment was done and the data was noted in the prescribed proforma. • Diet restriction was strictly followed during the period of drug administration as well in the dietary advice form • During the study period, no adverse reactions was reported due to the drug or disease. • The prognosis was assessed clinical assessment done on initial day and the end of the 46th day. The prognosis has recorded through PASI Score. • The outcome has clinically observed by PASI score, that was among 30 Children, 80% (24) were shown Good and 13% (4) shown moderate improvement and 7% (2) were shown mild improvement. • Statistical analysis was done for PASI Score, before and after treatment has been Significantly reduced. (p= 0.0001) • Stress has been relieved by reducing the lesions, itching and scaling. Academic performance has improved. CONCLUSION: From this study, Vallarai Nei (internal) and Sembarithipoo Ennai (External) is safe, efficacious and cost-effective, potent herbal drug in the treatment of KalanjagaPadai (Psoriasis). ➢ The ingredients of Vallarai Nei and Sembarithipoo Ennai has Antipsoriatic, Anti-inflammatory, Anti pruritic activity and Immunomodulator activity. ➢ The PASI score concluded, that was among 30 Children, 80% (24) were shown Good and 13% (4) shown moderate improvement and 7% (2) were shown mild improvement. ➢ Statistical analysis was done for PASI Score, before and after treatment has been Significantly reduced. (p= 0.0001).Therefore the trial drug Vallarai Nei is clinically effective. ➢ There was no adverse reaction reported during the trial period. ➢ The study maybe further carried out with the same drug “Vallarai Nei” and “Sembarithipoo Ennai” in a larger clinical population because of the favourable clinical outcome.
