Abstract

Evaluation of the drug Silethuma sura kiyalith kanji was done after getting approved by IEC of National Institute of Siddha. The IEC approval was obtained No: NIS/IEC/2019/D-25;13-09-2019. The trial was registered in Clinical trial Registry of India with Reg No. CTRI/ 2020/06/026170. • The Authentication of ingredients of the trial drug was obtained from medicinal Botanist NIS Chennai. • Purification of raw drugs and preparation of trail drug was done at Gunapadam Laboratory, Department of Gunapadam, NIS, Chennai. Bio Chemical Analysis of trial drug was done in Biochemistry Laboratory, Department of Biochemistry, NIS Chennai. Physiochemical and phyto chemical analysis was done in Noble Research Solutions, Chennai. • The drug Silethuma sura kiyalam with kanji was in the consistency of hard fibrous powder with brownish in color with strongly aromatic in odour and raw course in nature. • The total ash was found to be 3.8% which indicates the inorganic content of the drug. The value of acid insoluble ash was found to be 1.42% which indicates that the drug contains negligible amount of siliceous matter. The water soluble extractive value and alcohol soluble extractive value were found to be 15.39% and 4.59%. • HPTLC was done to identify phyto- chemicals and their Rf values were calculated. • The drug is free of microbial contamination. • The Bio chemical analysis of trial medicines showed shows presence of Aluminium, calcium, iron, zinc and magnesium. • Phytochemicals such as Alkaloids, steroids, coumarin, saponins and phenol, tannin, protein, betacyanin are present in the trial drug. • Pharmacological analysis of the drugs shows Anti pyretic actions. • The medicine chosen for this clinical trial is Silethuma Surakiyalam with kanji internally twice a day for 3 days. All the patients were kept under diet control during the treatment. • The clinical efficacy of the drug was analyzed statistically on all the symptoms based on the assessment criteria. The observation made during the clinical study showed that the trial drug Silethuma sura kiyalam with kanji was clinically effective. • Kabasuram is one among the acute infectious disease and it has been characterized as chills, fever, malaise, cough with or without expectoration, wheezing, vomiting associated with sputum, chest congestion or pain, arthralgia in Pillaippini Maruthuvam textbook. These symptoms are more or less similar to that of acute bronchitis. • Among 30 case, 30(100%) cases had fever, 25(83.33%) cases had difficulty in breathing, 29(96.66%) cases had cough, 28 (93.33%) had sneezing, 27(90%) had malaise, 4(13.3%) had polyarthralgia, 26(86.66%) cases had running nose, 15(50%) cases had wheezing. • At the end of the treatment, Fever was reduced in 100% of cases, difficulty in breathing was reduced in 26.66%, Cough wsa reduced in 30%, sneezing was reduced in 20%, malaise was reduced in 3.33%, polyarthralgia was reduced in 0% of cases, Running nose was reduced in 87.5% and wheezing was reduced in 26.66% of cases. • From the above results it is evident that signs and symptoms explained in Siddha and modern literatures are mostly reflected and in coherence with in the paediatric population. • The observation made during the clinical study showed that the trail drug, Silethuma sura kiyalam with kanji was clinically effective on Kabasuram (Acute bronchitis). No drug reactions were reported during the study. CONCLUSION: All the 30 patients of Kabasuram were treated with Silethuma sura kiyalam with kanji internally two times in three days. Kiyalam and Kanji (food) form of medicine is very easy to administer in children and also it assimilates quickly. The ingredients of Kabasura kudineer are affordable, easily available and harmless to children. The medicine has many properties to control the signs and symptoms of Kabasuram. During the course of treatment, no adverse effects were observed. Because of the efficient results clinically, the study may be carried over to further researches and it may motivate the upcoming generation to manage the disease Kabasuram through the Siddha medicine and also this study throw new glitters for the Siddha scientific committee. The present clinical study has established that Silethuma sura kiyalam with kanji is having sgood result in reducing the majority of symptoms and a sign of the Kabasuram.This has, in turn, provided a golden opportunity for new drug established in the management of Kabasuram.
