Abstract

BACKGROUND AND OBJECTIVES: Gingival health and colour are essential components for a charming smile. Gingival hyperpigmentation caused due to excessive melanin production is a common aesthetic concern for patients, especially in case of patients with gummy smile. Although hyperpigmentation of gingiva is not a medical problem, it can cause psychological disturbances in few patients. Various techniques have been proposed for depigmentation procedures. The aim of the present study is to compare the efficacy of Scalpel, diode laser and soft tissue trimmer in the management of gingival hyperpigmentation. METHODS: Thirty patients with unaesthetic gingival hyperpigmentation in the age group of 18-30 were randomized into three treatment groups. The clinical parameters assessed were Dummett oral pigmentation index (DOPI), Hedin melanin index (HMI), Bleeding, Pain & Wound healing. Image intensity analysis was alone done using Image J software to see the intensity changes objectively at baseline, 1, 3 and 6 months follow up. RESULTS: All three gingival depigmentation techniques in our study showed promising aesthetic results. Scalpel technique causes more bleeding during the procedure when compared to the laser and soft tissue trimmer technique. Pain was more in scalpel group compared to the soft tissue trimmer group at 1st and 4th day post operatively. Patients were less apprehensive with the use of laser technique since there is no bleeding and pain following the depigmentation procedure. Both scalpel and soft tissue trimmer showed faster healing when compared to laser technique. Clinical assessment of repigmentation at 1st, 3rd and 6th month showed that the scalpel group had least repigmentation eventhough the difference was not statistically significant. However the Image J intensity analysis showed better results in scalpel group when compared to other group which was statistically significant. CONCLUSION: In our study all three treatment groups showed improvement in depigmentation results from baseline to 6 month follow-up, but the scalpel technique showed marginally better results when compared to laser and bur abrasion methods with lowest rate of repigmentation and its cost effective.
