Abstract

RATIONALE: Poor working conditions of spinning mill workers make them indulge in adverse oral habits like smoking, chewing tobacco and alcohol. There is paucity of literature assessing these adverse habits and its oral health effects in this vulnerable population. AIM AND OBJECTIVES: The objectives of this study were to assess the prevalence of Potentially Premalignant Oral Epithelial Lesions (PPOEL) using World Health Organization-International Agency for Research on Cancer (WHO-IARC) guidelines for oral cavity examination and Toluidine Blue vital staining, and also to assess the risk factors for PPOEL with the help of a pre-tested questionnaire. MATERIALS AND METHOD: A cross-sectional study was carried out among 850 spinning mill workers of Coimbatore selected through cluster random sampling method. Data regarding their socio-demographics and adverse oral habits were collected through a pre-tested questionnaire using face-to-face interviews. Oral cavity examinations were carried out for the presence of premalignant lesions using WHO IARC guidelines and toluidine blue vital staining technique. Data on risk factors for oral premalignant lesions were also collected using the pre-tested questionnaire. RESULTS: The age of the participants ranged from 18-58 years and the mean age was 34±4 years. About53% (452) participants belonged to 31-40 years. 40% (337) of the participants were males and 60% (513) were females. Of the total population, 55% (465) belonged to lower middle class, 44% (374) were native to other districts of Tamil Nadu and 23% (195) were native to other States of India. The prevalence of premalignant lesions among the study participants was 19% (161). Out of the 161 participants with premalignant lesion, 29 had leukoplakia, 4 had erythroplakia, 10 had erythroleukoplakia, 26 had lichen planus and 92 had Oral Submucous Fibrosis. Of the total 850 participants, 71% (604) were never smokers, 3% (26) were past smokers, 26% (220) were current smokers. When asked about the history of smokeless tobacco use, 66% (561) were never users, 3% (23) were past users and 31% (266) were current users of some form of smokeless tobacco. About 70% (591) were abstainers of alcohol, 10% (85) were occasional drinkers, 7% (61) were frequent drinkers and 13% (113) were regular drinkers. About 30% (257) of the study population had a positive history of frequent consumption of hot and spicy food, 2% (21) had a positive history of frequent oral trauma, 22% (187) had a positive history of frequent oral ulcers, 65% (552) had a positive history of infrequent consumption of fruits and vegetables, 1% (8) had a positive family history of oral cancer and 7% (57) had a positive family history of other cancers. The prevalence of leukoplakia, erythroplakia, erythroleukoplakia and oral submucous fibrosis was higher among combined tobacco users compared to only smokers or only smokeless tobacco users. Lichen planus was predominantly higher among smokeless tobacco users compared with only smokers or combined tobacco users. CONCLUSION: As Oral Premalignant Disorders and other non-communicable diseases share common risk factors, existing programs can be easily remolded or a new programs targeting both can be implemented. PUBLIC HEALTH SIGNIFICANCE: Assessment of the prevalence and risk factors of oral premalignant lesions among spinning mill workers help us frame policies and programs.
