Abstract

OBJECTIVES: To describe the maternal and perinatal outcomes in patients with Placenta Previa and previous LSCS. MATERIALS AND METHODS: This is a Retrospective and Prospective Cohort study conducted in the Department of Obstetrics and Gynaecology in Government Kilpauk Medical College and Hospital from July 2019 to September 2020. A total of 120 patients were divided into 2 groups, 60 Previous LSCS with Placenta Previa and 60 Previous LSCS without Placenta Previa. The Maternal and Perinatal outcomes were compared between the 2 groups. RESULT: Placenta Previa was common in the age group 25 – 30 years. Marginal Placenta Previa was the most common type(41.7%). All the cases delivered before 37 weeks and all were delivered by caesarean section. Previous LSCS and previous history of abortions were the major risk factor in this study. There was a significant association between Placenta Previa and passive smoking (3.5 times). Antepartum haemorrhage, post partumhaemorrhage, internal iliac artery ligation, blood transfusion, peripartum hysterectomy, post operative elective ventilation and puerperal sepsis were all significantly higher in the study group. 73% of the babies were low birth weight and 5% of the babies had congenital anomalies. No maternal and perinatal mortality was seen in both the study and control group. CONCLUSION: Previous LSCS was the one of the major risk factor for Placenta Previa, efforts should be made to reduce primary LSCS rate. Medical methods of abortions should be practiced in most of the cases instead of dilatation and curettage. Training must be given to the health care professionals in visualizing the placental position via ultrasound and in performing various life saving procedures.
