This study was designed to have a complete contemporary understanding of two different Siddha formulations CC and PT in management of īrpīṉicam using various Siddha and bio medical parameters. The ingredient of drug catakuppai were collected from Krishnagiri district and other ingredients are from Tirunelveli district and authenticated by taxonomist in Government Siddha medical college and hospital, Palayamkottai. After that, the drug was prepared as per Siddha texts in Government Siddha medical college, Palayamkottai. The prepared drug was authenticated by the trained experts from Gunapadam department of Government Siddha medical college and hospital, Palyamkottai for its completion. After that, the prepared drug was qualitatively analysed by various chemical methods, which showed the presence of calcium, sulphate, chloride, unsaturated compound. Then antimicrobial activity of prepared drug was evaluated against the Klebsiella pneumonia, Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus, Streptococcus pneumoniae indicates that CC and PT was effective only against Klebsiella pneumonia. Apart from this the therapeutic effect of anti-inflammatory and antihistamine activity on CC was evaluated using carrageenan induced anti-inflammatory model in male wistar rats and histamine induced bronchoconstriction in guinea pig. After the preclinical study, the clinical evaluation of affected subjects with īrpīṉicam symptoms in 7 to 12 age groups was done in OPD and IPD. The prognosis of affected subjects was assessed based on general criteria, Siddha parameters, causes and signs and symptoms (SNOT-22). More or less equal number of male and female subjects were reported with īrpīṉicam symptoms predominantly in kūtirkālam and muṉpaṉikālam. From this work, the study drug is significantly effective in most of īrpīṉicam affected individuals in acute stage compared with chronic stage. However, in repeated infective subjects took more days to recover. Hence specific studies needed in based on every particular causative factor for better understanding of the drug. As this study was in minimum number of samples furthermore long-term studies have to perform under influence of variations in īrpīṉicam, so that we can gain complete information on CC and PT activity in īrpīṉicam affected individuals.
