The aim of this study was to assess the efficacy of the trial drug Adathodai Rasayanam for the management of Soolikanam (Childhood asthma) without any harm to the children. The symptoms of bronchial asthma nearly correlate with the Soolikanam which is described in Siddha literature Balavagadam. The literature evidence of both Siddha and Modern literatures were reviewed. The reviews of literature and recent studies on the individual drug of Adathodai Rasayanam were collected. This clinical study has been approved by IEC of National Institute of Siddha and the trial was registered in Clinical Trial Registry of India. Children who visited the Kuzhandhai Maruthuvam OPD of National Institute of Siddha with the symptoms of Soolikanam were clinically diagnosed and recruited for the study based on inclusion and exclusion criteria. Before recruitment parent was informed about the study and consent was obtained from the parent. The Authentication of ingredients of the trial drug was obtained from the Medicinal Botanist, National Institute of Siddha. Purification of raw drugs and preparation of trial drug was done in Gunapadam Laboratory, Department of Gunapadam, National Institute of Siddha, Chennai. Biochemical Qualitative analysis of trial drug was done in Biochemistry Laboratory, National Institute of Siddha, Chennai. Physicochemical analysis of Adathodai Rasayanam was done in Noble research solutions, Chennai. The Biochemical analysis of Adathodai Rasayanam revealed the presence of sulphate, phosphate, carbonate, iron, calcium, arsenic, reducing sugar, alkaloids and tannic acid. Phytochemical analysis of Adathodai Rasayanam revealed the presence of alkaloids, steroids, triterpenoids, phenol, tannin saponin and sugar. The presence of these phytochemicals enhances the activity of Adathodai Rasayanam. The review of ingredients of Adathodai Rasayanam had anti-inflammatory, antianti-tussive, anti-oxidant and anti-asthmatic activity. Therefore, with the above benefits the trial drug Adathodai Rasayanam can be considered as an effective medicine for Soolikanam (Bronchial asthma). The medicine chosen for the management of Soolikanam was Adathodai Rasayanam with the dosage of 170 mg for 3 to 4 years children and 500 mg for 5 to 7 years children three times a day. The medicine was given for 45 days. During the course of treatment, acceptance of medicine was good and the children did not show any adverse effects. Hence, the drug was believed to be safe and effective in the management of Soolikanam. The outcome of this study was clinically observed by PASS score and PEFR. After 45 days of treatment, among 30 children 22 patients (73.3%) had good improvement, 5 patients (16.7%) had moderate improvement and 3 patients (10% ) had poor improvement.PEFR was measured for 18 children and among those 11 patients (61.1%) had good relief,4 patients (22.2%) had moderate relief and 3 patients (16.7%) had mild relief. There is a highly statistical significance(p<0.05) differs in PASS score and PEFR in the management of Soolikanam after medication of Adathodai Rasayanam. CONCLUSION: In this study the trial drug Adathodai Rasayanam was administered to the children of age group 3-7 years affected with Soolikanam (bronchial asthma). Thirty children of Soolikanam were given Adathodai Rasayanam internally for 45 days. The ingredients of Adathodai Rasayanam are easily available and affordable to all people. During the course of treatment all children never hesitated to take the medicine due to its palatability. Most of the drugs of Adathodai Rasayanam had anti- inflammatory, Immunomodulatory, anti-asthmatic and anti-microbial property which revealed the properties to manage the symptoms of Soolikanam. No adverse reactions were observed during the course of treatment. After 45 days of treatment children were improved clinically with good prognosis. The efficacy of trial drug measured by PASS assessment was highly significant(p<0.005) which was showed by statistical analysis. So, the author recommends that this trial drug Adathodai Rasayanam can be administered to the children for the management of Soolikanam (Bronchial asthma). By the encouraging study results, this drug may be taken for larger population to establish the novelty of this medicine further.
