Abstract

BACKGROUND: Cigarette smoking is a significant risk factor in the pathogenesis and progression of periodontal disease, leading to an increase in periodontal tissue destruction as a consequence of altered production of cytokines, inflammatory markers and host-cell function. Smoking can impair various components of the host immune response and immune system. The virulence factors of periodontal pathogens stimulate inflammatory cytokine expression by mononuclear cells. IL-β_ and IL-8 are the key mediators involved in the pathogenesis and disease progression. AIM OF THE STUDY: To evaluate the effect of non-surgical periodontal therapy on serum levels of IL-β_ and IL-8 in smokers and non-smokers with chronic periodontitis before and after scaling and root planing. OBJECTIVES: 1. To find out the effect of non-surgical periodontal therapy on clinical parameters like plaque score, gingival score, recession, probing pocket depth and clinical attachment level in smokers and non-smokers with chronic periodontitis before and after scaling and root planing. 2. To evaluate the serum levels of IL-β_ and IL-8 in smokers and non-smokers with chronic periodontitis after scaling and root planing. MATERIALS AND METHODS: This was a comparative interventional study including a total of 52 (26 smokers with chronic periodontitis and 26 non-smokers with chronic periodontitis) subjects. Clinical parameters (GI, PI, recession, PPD and CAL) were recorded at baseline and 4 weeks after initial periodontal therapy. Initial periodontal therapy included oral hygiene instructions and full mouth scaling and root planing. Five millilitre of venous blood sample was collected from each subject at baseline and four weeks after initial periodontal therapy to evaluate serum IL-1_ and IL-8. This biochemical parameters were assessed using ELISA sandwich method. RESULTS: Smokers with chronic periodontitis showed statistically significant lower GI and higher PI, gingival recession, PD and CAL as compared to non-smokers with chronic periodontitis. Statistically significant reduction in periodontal parameters were seen in both groups after periodontal therapy. IL-β_ and IL-8 were increased in both groups at baseline, after SRP both groups showed statistically significant reduction in IL-1_ and smokers with chronic periodontitis showed statistically significant increase in IL-8 after SRP. CONCLUSION: Serum levels of IL-β_ and IL-8 are elevated in smokers and non-smokers with chronic periodontitis. This study shows that initial periodontal therapy contributes statistically significant improvement in clinical parameters and biochemical parameters excluding IL-8. Further controlled trials with larger samples are needed to confirm this finding.
