Abstract

BACKGROUND AND AIM OF THE STUDY: Central neuraxial analgesia is the gold standard technique and the versatile method of labour analgesia technique for pain control in obstetrics. Several exciting advances has been developed in the field of neuraxial analgesia in terms of refinement of techniques. The aim of this study is to compare the efficacy of labour analgesia with dural puncture epidural versus standard epidural technique. METHODS: This study was conducted in 50 laboring parturients belonging to ASA I and ASA II category without any comorbidities. Parturients were divided into two groups of 25 each. Group A receives traditional Epidural technique and Group B receives Dural Puncture Epidural technique. Both groups received epidural bolus of 12 ml of 0.125% bupivacaine with 50 mcg fentanyl followed by infusion of 0.1% bupivacaine with 2mcg/ml fentanyl every 90 minutes. Time to achieve adequate analgesia, block quality and occurrence of side effects were compared between both the groups. RESULTS: In group A the mean time to achieve adequate analgesia was 11.60 minutes and in group B the mean time to achieve adequate analgesia was 10.04 minutes. In group A the percentage of parturients with VAS score <10 at 10 minutes following epidural bolus administration was 52% and in group B the percentage of parturients with VAS score <10 at 10 minutes was 60%. CONCLUSION: On comparing traditional epidural technique with dural puncture epidural technique for labour analgesia, time to achieve adequate analgesia is faster in dural puncture epidural technique than traditional epidural technique. Dural puncture epidural improves the sacral spread, onset of analgesia and bilateral pain relief in laboring women with very few maternal and fetal side effects and with no negative affect over the mode of delivery.
