Abstract

BACKGROUND: Chronic Kidney Disease (CKD) is a disease of global burden typically characterised by progressive loss of nephron function ultimately leading to end stage renal disease (ESRD) characterized by hypertension, anemia, renal bone disease, nutritional impairment, neuropathy, impaired quality of life and reduced life expectancy ultimately needing some form of renal replacement therapy. This study is taken up to analyse the course of CKD in patients getting admitted in a tertiary care centre. The aim of the study is: 1. To follow-up CKD patients and estimate e-GFR. 2. To arrive at statistical data on influence of co-morbidities (SHTN, DM) on rate of decline of e-GFR on the disease process. 3. To know their influence on rate of decline of e-GFR. METHODS: After ethical approval, the study was conducted in patients with CKD who were attending the OPD during the 6 months study duration. All patients of GFR ≤ 59mL/min/1.73m² fulfilling KDIGO guidelines were included in this study. Patients with other comorbid illnesses like Liver disorders, Rheumatological Disorders, HIV seropositivity, Malignancy, chronic bed ridden patients as well as pregnant patients were excluded from this study. Thorough history taking, clinical examination and laboratory investigations were DONE. The data collected were tabulated and statistical analysis were done. RESULTS: In this study around 18.4% had kidney disease; 17.6% had hypertension and 16.9% had diabetes as an etiology and 18.4% had both diabetes and hypertension as an etiology. There was statistically significant association of eGFR values with the age of the patients, diabetic status, hypertensive status, dyslipidemia and drinking alcohol. CONCLUSION: The factors affecting the eGFR in Chronic Kidney Disease patients were age, diabetic status, Hypertensive status dyslipidaemia, cardiovascular status, smoking and alcoholism. Although there are many risk factors and varied etiology for Chronic Kidney Disease, diabetes and hypertension are the two major chronic non communicable diseases that results in the progression of Chronic Kidney Disease.
