Abstract

OBJECTIVES: The purpose of this study is to evaluate for the ease of surgical access for plate fixation and the intraoperative surgical time and to assess the postoperative fracture reduction and plate adaptation clinically and radiologically between transoral and transbuccal approaches for management of mandibular angle fractures using single non compression miniplates. MATERIALS AND METHODS: In this prospective study 10 patients with unilateral mandibular angle fractures were included and divided randomly into 2 groups containing five patients in each group. All patients were treated under general anesthesia, using a single 2.0 mm 4-hole with a gap miniplate and secure with monocortical screws through transoral and transbuccal approach. The parameters evaluated were intraoperative time, surgical site accessibility, occlusal discrepancy, wound infection / dehiscence and Radiographic evaluation of fracture reduction. The data was tabulated and subjected to statistical analysis. P -value less than 0.05 was considered significant. RESULTS: No significant difference was seen between the two groups for intraoperative time and surgical site accessibility. Radiographic interpretation of fracture reduction revealed statistical significance for Group B from points C to D as compared to Group A. No cases of malunion/non-union were noted. Infection was noted in 1 patients in Group B compared to 2 patients in Group A. Minorocclusal discrepancy was observed in 3 patients in group A and 1 patient in Group B postoperatively and was successfully managed by guiding elastics. The small sample size and limited follow-up period could be considered as the limitation of the study. CONCLUSION: The transbuccal approach was superior to the transoral approach with regard to radiographic reduction of the fracture gap, inconspicuous external scarring, and fewer postoperative complications. We preferred the transbuccal approach due to ease of use, minimal requirement for plate bending, and facilitation of plate placement in the neutral mid-point area of the mandible. The small sample size and limited follow-up could be considered as the limitations of this study.
