Abstract

INTRODUCTION: This study was carried out to determine the variations that occur in the intraocular pressure (IOP) of eye, with Metabolic acidosis (MA), a disorder of acid-base balance, frequently encountered in emergency and critical care units. Metabolic acidosis is the commonest acid-base imbalance that is characterized typically by a reduction in bicarbonate (HCOз(-)), along with compensatory reduction in the Pco₂ (arterial partial pressure of carbondioxide) and a markedly low or subnormal ph. It occurs due to increased acid formation/diminished acid excretion or due to loss of bicarbonate from the body, especially when it exceeds the body’s adaptive mechanism to handle this. AIM OF THE STUDY: To evaluate intra ocular pressure (IOP) variation in patients diagnosed with Metabolic acidosis (MA) and to compare it with Age matched controls at PSGIMSR during a period of 18 months. MATERIALS AND METHODS: The criteria for inclusion were patients > 20 years of age, who were diagnosed with MA of any cause (cases) and patients visiting ophthalmology OPD (controls). We excluded Patients who had already been treated for MA, who are being treated for Glaucoma and terminally ill patients. There were a total of 200 patients satisfying the above criteria. Informed consent from patients’s attenders, treating doctors and patients was obtained. Anterior segment evaluation was done using torch light. Topical anaesthesia with 0.5% proparacaine eye drops and staining of cornea with 1mg fluorescein sodium/strip was done prior to taking IOP. IOP was measured in patients diagnosed with metabolic acidosis and after treatment of metabolic acidosis (cases) and also in age-matched controls using Perkins tonometer. Readings documented were ABG (arterial blood gas analysis) report and IOP of cases and controls. Results were compared and analysed using appropriate statistical tests. Study was approved by the Institutional human ethics committee. RESULTS: The results obtained indicate that IOP may decrease at the time of MA and returned to baseline following correction of MA. CONCLUSION: Our observation suggests that MA, a frequent acid-base imbalance may induce a decrease in the IOP and that the treating physician must be aware of this, so that usage of topical anti-glaucoma medications may not be compelled in such situations.
