Abstract

INTRODUCTION: Radial head fractures are the most common fractures of elbow with an incidence of 2.5 to 2.9 per 10,000 people per year.1 Radial head fractures are found to be more common in women than men and most frequently occur between ages of 20 and 60 years. Undisplaced and minimally displaced radial head fractures typically occur as isolated injuries while more displaced and comminuted fractures commonly have associated injuries to the collateral ligaments and may have associated fractures of the coronoid, capitellum, or proximal ulna. In high energy trauma, dislocations of the elbow and/or forearm can also occur. Disruption of the interosseous membrane and distal radial ulnar joint ligaments may result in axial instability of the forearm, termed the Essex–Lopresti lesion. The joint between the radial head and the capitellum is important in maintaining the stability of axial and valgus loading of the forearm. The radial head is increasingly recognized as an important stabilizer of elbow. The majority of radial head and neck fractures are minimally displaced and are isolated injuries. These fractures typically have good functional outcome with nonsurgical treatment. The optimal management of displaced radial head fractures has not been established. PURPOSE: This study is conducted to analyse the functional and radiological outcome in patients with fracture of head of radius, managed by radial head excision with those who were managed by radial head prosthetic replacement and to interpret the better treatment modality among the two surgical procedures. AIM OF THE STUDY: To manage the fracture of head of radius by radial head excision in one group of patients. OBJECTIVES: 1. To manage the fracture of head of radius by radial head prosthetic replacement in the other group of patients. 2. To evaluate the functional and radiological outcome in patients with fracture of head of radius, undergoing radial head excision and radial head prosthetic replacement. 3. To compare and interpret the better surgical procedure among the two above mentioned surgical procedures for patients with fracture of head of radius. METHOD: This study is a Randomized Controlled Trial of 32 patients with fracture of head of radius. The patients were divided into 2 groups comprising of 16 patients in each group. One group of patients were treated with radial head excision and the other group with radial head prosthetic replacement. Outcomes were assessed at every follow up. RESULT: The radial head prosthetic replacement group had better elbow stability, patient compliance, less complications and were able to return early to work, when compared with that of radial head excision group. CONCLUSION: Considering early return to work, better stability of elbow, less complications and better ability to perform daily functions, the radial head prosthetic replacement would be a better surgical procedure when compared with that of radial head excision depending on the type of fracture and the associated injuries.
