Abstract

INTRODUCTION: Carcinoma breast is one of the most common cancer among female population, curative treatment rate for the same has been significantly rising due to multimodality screening and management. Among the complications that develop from treatment methods, seroma (Also called lymphocele) and lymphedema occur as a consequence of axillary treatment of breast cancer. Hence there is a need to study various factors predisposing to the increase in the incidence of seroma and lymphedema. AIM AND OBJECTIVES: 1. To Estimate the incidence of seroma and lymphedema in breast cancer patients who are subjected to surgery and radiotherapy involving the axilla. 2. To determine the factors which predispose to the incidence of seroma and lymphedema. MATERIALS AND METHODS: Patient admitted in TVMCH for the management of Carcinoma breast from November 2017 to May 2019 are monitored from the day of admission. A prospective study with a sample size of 83 patients is done. Patients diagnosed as carcinoma breast and planned for axillary treatment, patient who have undergone axillary dissection for ca breast now attending to hospital for chemotherapy and radiotherapy, patient who have undergone axillary dissection and adjuvant radiotherapy now attending to hospital for follow up are included in this study. All patients are subjected to pre treatment and post-treatment anthropometric measurement, details of the type of surgery done, details of the postoperative care including placement of drains and range of motion arm exercise, details of radiotherapy given are collected and analyzed. RESULTS: Increased incidence of seroma is observed in. Age > 50 years, Bmi > 29, Size of tumor > 5cm. More than 16 nodes excised during axillary dissection. Increased risk of lymphedema is observed in patients who Underwent concurrent axillary dissection harvesting more than 16 nodes and axillary radiotherapy, Obesity, Patients who developed Seroma formation after surgery. CONCLUSION: We have arrived at a conclusion that some of the risk factors entailed in the causation can be eliminated to decrease the incidence of seroma and lymphedema. Notable interventions which showed promising results are Conservative breast surgery, Sentinel lymph node dissection, tailoring radiotherapy to indicated cases and only at required fields, Adequate and comprehensive nodal isolation during histopathological examination. However, further research is still required to provide better outcomes regarding the treatment protocols in order to improve the quality of life of patients.
