Abstract

INTRODUCTION: The word “Jaundice” arises from French word Jaune which means yellow. Jaundice is defined as yellowish discolouration of the skin, sclera and mucus membrane by bilirubin – yellow orange bile pigment. Jaundice is a most common problem in surgical practice. Its etiology can often be made correctly but usually biochemical and radiological imaging investigations are required because of a variety of etiologies and presenting features. This study is an aetio pathological analysis & presentation of various causes of obstructive jaundice, correlation of biochemical and radiological investigations and morbidity and mortality due to the disease. It also emphasizes on the surgical management that could improve survival of patients admitted in our hospital with jaundice. AIMS & OBJECTIVES To estimate the correlation of clinical, biochemical and radiological features in obstructive jaundice patients. 1. To analyze the efficacy different modalities of treatment and other complications. 2. To assess the morbidity and mortality of the disease MATERIALS AND METHODS: This study was conducted in Department of General Surgery, Tirunelveli Medical College Hospital, Tirunelveli for a period of two years from September 2017 to August 2019 . RESULTS: Choledocholithiasis is the most common aetiology of obstructive jaundice and most common age group affected by obstructive jaundice is 41-50 years. Females are affected more than males. Males are commonly affected by malignancies than females, among malignancies periampullary carcinoma is most common. Obstructive jaundice commonly presented with abdomen pain f/b jaundice. Among radiological investigations MRCP plays a vital role. Majority (80%) of patients treated surgically and most common complication in post operative period is wound infection (10%) that too treated successfully by non surgical interventions. CONCLUSION: Obstructive jaundice is a common surgical problem in our setting and poses diagnostic and therapeutic challenges. It is more common among females. Benign etiology is common in females and malignant etiology is common in males Careful clinical examination and correlation with radiological investigations played a major role in diagnosis and treatment Pre operative evaluation and correction of abnormalities will reduce the morbidity and mortality. Minimal invasive surgery will play a role in surgical management of obstructive jaundice.
