Abstract

Blunt trauma accounts for more than 80% in most trauma centres. Significant abdominal trauma is present in 12-15% in association with other system injuries. Although operative management is required in 30-40% of blunt injuries of the abdomen, prompt evaluation of the need for conservative management is important. The purpose of this study was to evaluate non operative management in patients with blunt injury abdomen with solid organ injuries and the impact and effectiveness of non operative management in high grade solid organ injuries. Injury to the solid abdominal organs such as the liver, spleen, kidney, and pancreas is one of the mostcommon injury patterns management of blunt injuries can be challenging. During the past three decades, there has been a major shift from operative to increasingly non-operative management of traumatic injuries. Greater reliance on non-operative, or "conservative" management of abdominal solid organ injuries is facilitated by the various sophisticated and highly accurate non-invasive imaging modalities at the trauma centre. This is to compare and evaluate the effectiveness of non operative management in low grade and high grade blunt solid organ injuries abdomen PROTOCOL: Summary and Background: To evaluate non operative management in patients with blunt injury abdomen with solid organ injuries and asses their impact and effectiveness of non operative management in high grade solid organ injuries. AIMS AND OBJECTIVE: To evaluate the outcomes in non-operative management in patients with blunt injury abdomen with solid organ injuries and impact and effectiveness of non operative management in high grade solid organ injuries MATERIALS & METHODS: STUDY DESIGN: Prospective cohort study. METHODOLOGY: All patients admitted with blunt injury abdomen with solid organ injury in Coimbatore Medical College Hospital between January 2018 and July 2019 were taken into the study. Hemodynamically unstable patient, hollow viscus perforation,peritonitis patients to be excluded by thorough clinical examinations ,radiological investigations.  Solid organ injuries graded according to AAST grading  Hemodynically stable patient observed by clinically radiologically by monitoring pulse,BP,Hb,free fluid level in abdomen.if condition worses patients will be managed operatively  The impact of non operative management and causes for converting to operative management and their post operative outcomes were analysed and assesed. INCLUSION CRITERIA:  All patients more than 12 years, admitted with blunt injury abdomen with solid organ liver, spleen, kidney injury in Coimbatore Medical College Hospital  Hemodynamically stable patient [no tachycardia, nohypotension, pt not in shock] EXCLUSION CRITERIA:  Hemodynamically unstable patient.  Patient with hollow viscus perforation, peritonitis.  Patients aged less than 12 years.  Patients with penetrating trauma.  Patients with head injuries.
