Abstract

BACKGROUND: GOITRE is defined as generalised enlargement of thyroid swelling Patients with goitre may be asymptomatic, or may present with compressive symptoms such as cough or dysphagia. Goitre may also present with symptoms due to associated hypothyroidism or hyperthyroidism. Thyroid stimulating hormone is the appropriate first test for all patients with goitre; if this hormone is low a radionuclide scan is helpful. Thyroid ultrasound has become an extension of physical examination and should be performed in all patients with goiter. A goiter is an enlarged thyroid gland, and it may be diffuse or nodular. A goiter may extend into the retrosternal space, with or without substantial anterior enlargement. Because of the anatomic relationship of the thyroid gland to the trachea, larynx, superior and inferior laryngeal nerves, and esophagus, abnormal growth may cause a variety of compressive syndromes. Thyroid function may be normal (nontoxic goiter), overactive (toxic goiter), or underactive (hypothyroid goiter). AIM OF THE STUDY: 1. To find out the incidence of malignancy among Goitre. 2. To find out the different types of malignancy among Goitre. 3. To evaluate the risk factors associated with occurrence of malignancy in a Goitre. MATERIALS AND METHODS: This is a prospective study of randomly selected patients with clinically palpable, Goitre diagnosed and treated at Chengalpattu medical college Total duration of study was two years, from 2017 DECEMBER to 2019 DECEMBER. Each patients Symptoms and signs were entered in a proforma with detailed clinical examination in relation to the thyroid swelling and lymph node involvement and a routine systemic and general examination was done. All patients were subjected to basic investigations like complete hemogram, Blood sugar, Blood urea, serum cholesterol, urine analysis, chest radiogram and radiogram of neck. Tissue diagnosis was obtained by fine needle aspiration cytology in all the patients. Thyroid profile was done in selected patients with features of hyper or hypofunctioning of the gland. Radio isotope scan was not done since the facility was not available at our hospital. All operated specimens were subjected to Histopathological examination. Type of study: prospective study. Study approval: Prior to commencement of this study, Ethical Committee of Chengalpattu medical college had approved the thesis protocol. Place of study: Chengalpattu medical college hospital. Period of study: Two year duration starting from December2017 to December 2019. Sample size: 160 cases. INCLUSION CRITERIA: Patient presenting with goiter, Age group 20-60, Both sex. EXCLUSION CRITERIA: Female patient not attained menarche, Age less than 19 years, History of radiation in childhood. CONCLUSION: 1. To study the incidence of malignancy among Goitre. 2. To study the different types of malignancy among Goitre. 3 .To study the risk factors associated with occurrence of malignancy in a Goitre.
