Abstract
BACKGROUND: Acute pancreatitis is a common and a highly challenging clinical condition encountered in our day to day surgical practice. Acute Necrotising pancreatitis being associated with high mortality, scoring systems has been devised to assess the severity. The Neutrophil-Lymphocyte Ratio (NLR) and platelet Lymphocyte ratio (PLR) calculated from the WBC differential count provides a rapid indication of the extent of an inflammatory process. AIM OF STUDY: To investigate if Neutrophil- Lmphocyte Ratio (NLR) and Platelet lymphocyte ratio can act as a early predictor of necrosis in acute pancreatitis. MATERIALS & METHODS: The NLR and PLR were calculated on 130 acute pancreatitis patients on day 0, day 1 and day 2 of admission. The NLR and PLR obtained were correlated with the CECT of pancreatitis patients. RESULTS: Of the 130 patients, 39 patients had necrosis in CECT abdomen. The NLR and PLR values calculated in acute necrotizing pancreatitis was significantly higher than in mild acute pancreatitis on all 3 days. CONCLUSION: Elevation of Neutrophil Lymphocyte Ratio (NLR) and Platelet lymphocyte ratio (PLR) during the first 48hrs of admission is significantly associated with necrosis in acute pancreatitis and can be used as a simple, single, cost-effective indicator in assessing necrosis in acute pancreatitis.
