Abstract

INTRODUCTION: Perforation is defined as a hole and break in the containing wall or membrane of an organ or structure of body. Perforation occurs when erosions, infections or other factors create a weak spot in the organ and internal pressure causes a rupture which results in peritonitis which is defined as an inflammation of the membrane which lines the inside of the abdomen and all of the internal organs; this covering membrane is called peritoneum. OBJECTIVES: To determine the prevalence of Helicobacter pylori infection in patients with gastro duodenal perforation peritonitis. PATIENTS AND METHODS: This prospective study was conducted patients who got admitted with gastro duodenal perforation peritonitis, in the Department of General Surgery, Government Rajaji Hospital, Madurai medical college, Madurai. All patients were evaluated by full history, clinical examination and relevant laboratory investigations including. X-ray chest and abdomen, ultrasound abdomen, RUT for H-Pylori, biopsy and histopathology, and culture for detection of helicobacter pylori infection at gastro duodenal perforation site. Statistical analysis were performed with IBM SPSS version 16 (SPSS Inc., Chicago, IL). RESULTS: During study period 50 patients were diagnosed as case of Gastro Duodenal perforation peritonitis. Male (86%) outnumber female (14%). Majority of patients were more than 30 years of age with mean age of 41.36 years SD ± 10.614. The RUT, histopathology, and culture for helicobacter pylori was positive in 39 (78%) cases. On exploratory laparotomy 8 patients had gastric perforation (16%), and 42 patients had duodenal perforation (84%). Among them 75% were positive for H.pylori infection on gastric perforation and 78.60% were positive for H.pylori infection on duodenal perforation. CONCLUSION: There appears to be an association between H. pylori infections and subsequent gastro duodenal perforation peritonitis.
