Abstract

Inguinal block dissection or groin dissection, has a major role in the management of patients with penile, vulval, anal, and cutaneous carcinomas. The inguinal lymphnodes are one of the major lymphatic blocks of the human body. They are responsible for drainage of the lower limbs, genitalia, posterior perineum, and lower part of the abdominal wall. The anatomy of the inguinal lymphatics informs a logical approach to the examination of the region that can readily be taught to patients at risk of inguinal metastasis from a range of cancers. Inguinal lymph dissection is associated with high postoperative morbidity, related to wound healing and lymphoedema. Meticulous surgical technique is needed to reduce postoperative wound complications. Various incisions are used for inguinal block dissection for prevention of wound complications. Here we compare conventional lazy ‘s’ incision versus horizontal incision for inguinal block dissection and its post operative wound complications in 50 cases in our GRH, Madurai. The reason for major wound complications is due to compromised vascular supply in skin flaps because Cutaneous vessels are horizontally oriented and compromised in vertical/lazy ‘s’ incision. Tension line are oriented horizontally in thigh, incision against these line would affect wound healing
