Abstract

BACKGROUND: Chronic anal fissure is a common and distressing disease, with an incidence higher than encountered. With constant research, the understanding of etiopathogenesis and management approach is evolving. The treatment method for this pathology must be optimal in clinical outcome, least painful and patient friendly. My study is to compare the proven lateral anal sphincterotomy with the local application of Diltiazem gel for fissure management. AIM OF THE STUDY: To compare the effectiveness of Lateral anal sphincterotomy and 2% Diltiazem gel local application in terms of effectiveness, complications and hospital stay. MATERIALS AND METHODS Prospective study conducted with the patients attending General Surgery Out Patient Department with H/o anal fissure for 6 months were enrolled for the study after institutional ethics approval and consent from study subjects. Patients between age 20 – 60 years were included and divided into 2 groups and subjected to surgical and medical management. The results were compared for efficacy. RESULTS: This study revealed that males were more commonly affected than females in the age group 41-50. Lateral anal sphincterotomy group had better relief of symptoms. 29% of DTZ group had to be intervened surgically to treat fissure. CONCLUSION: Our study concludes that most of the acute fissures heal spontaneously. Chronic fissure can be managed conservatively with 2% Diltiazem gel local application and persistent fissure should be treated with lateral partial anal sphincterotomy. Finally Diltiazem 2% gel can be used as first line of treatment for chronic anal fissure.
