BACKGROUND: Thyroid nodules are more prevalent in the general population accounting for 8.5%. In India prevalence of palpable thyroid nodule is 12.2%. The rate of malignancy in thyroid nodules ranges from 1.5% to 17%. Although the risk of malignancy in any thyroid nodule is less, thyroid cancer carries the main differential diagnosis. Thyroid nodules, comprise benign and malignant neoplasms, as well as non neoplastic lesions. The diagnosis of thyroid nodules can be established by histopathology in most of the cases. Some Thyroid lesions with nodular architecture and follicular pattern of growth often pose difficulties in accurate diagnosis. Identification of benign lesion and accurate diagnosis is very important for proper management. Here arises the need for immunohistochemistry in differentiating between nonneoplastic, benign and malignant thyroid lesions. Galectin 3, betagalactosyl bindinglectin helps in differentiating between non neoplastic, benign and malignant thyoid nodules. MATERIALS AND METHODS: All Nodular thyroid lesion with histopathological confirmation are taken. Galectin 3 was performed in 50 thyroid nodule cases. Totally out of 50 cases, 28 cases were non neoplastic lesions,8 cases were benign neoplasm and 14 were malignant neoplasmsExpression of Galectin 3 was studied. RESULTS: Galectin 3 was 100% (14 0ut 0f 14) expressed in thyroid malignancy, 25% (2 0ut of 8) in case of benign neoplasm and 33.14 %(9 out of 28 ) in case of non neoplastic lesions. 2+ is the best cut off for galectin3 to detect the malignancy of a nodule with a sensitivity of 85.7% and specificity of 97.2%., 2+ and above is the best cut off for galectin3 to detect the malignancy of a tumour with a sensitivity of 85.7% and specificity of 100%. CONCLUSION: Galectin 3 is a reliable and cheap marker of malignant thyroid nodules particularly for the diagnosis of papillary carcinoma of thyroid.Galectin 3 has high sensitivity and specificity to distinguish between benign and malignant thyroid nodules.
