Abstract

INTRODUCTION: Neuroendocrine neoplasms (NENs) are a heterogeneous group of tumours characterized by the presence of neurosecretory granules and characteristic histology and immunoprofile. OBJECTIVES: To quantitate mitotic activity using the mitoses specific marker, Anti-Phosphohistone H3 (PHH3) in neuroendocrine tumours of the hepatopancreaticobiliary system, grade the tumour and compare it to the WHO mandated tumour grading methods. METHODS: Neuroendocrine tumours of hepatopancreaticobiliary system were graded according to WHO 2017 and WHO 2019 classification into Grade 1, Grade 2 and Grade 3 tumours and also provisionally classified based on HE MI/MIB1 discordance into G1/G2 and G2/G3. Depending on morphology, G3 neoplasms were divided into small cell and MiNENs. Concordance between by HE MI, MIB1 and PHH3MI grades and correlation between continuous variables were analysed using weighted kappa statistics and Spearman’s correlation respectively. ROC and cut off analyses were done to determine optimal PHH3 MI cut off value. Interobserver agreement was given by Intraclass correlation coefficient. RESULTS AND CONCLUSION: PHH3 immunolabelling allows for quick and easy identification of mitotic figures. It had the highest concordance with MIB1 grade (i.e. overall WHO grade). There was a significant positive correlation between PHH3 MI and HE MI and PHH3 MI and MIB1 labelling indices. With optimal cut offs of 2 and 17 to distinguish between Grade 1 and Grade 2 and between Grade 2 and Grade 3 tumours respectively, there was an overall good sensitivity and specificity. Interobserver agreement was excellent.
