Abstract

BACKGROUND: Diabetes mellitus affects 2.5 % of pregnancies of which 65% are Gestational Diabetes Mellitus and 35% are complicated by preexisting diabetes mellitus. Nowadays the incidence of Gestational Diabetes mellitus is increasing such that 1 in every 5th pregnant women are likely to have GDM. GDM produces various maternal and fetal complications, hence this study tried to analyze the histomorphological changes of placenta associated with Gestational Diabetes. AIMS AND OBJECTIVES: To study the morphological changes in placenta in Gestational diabetes mellitus and compare the changes with patients in diet and insulin therapy and also to correlate the changes with special stain such as Periodic Acid Schiff, Masson trichome and immunohistochemistry using CD 34. MATERIALS AND METHODS: 15 placentas were collected from normal patients without any medical illness ,20 from Gestational diabetes mellitus patients on diet therapy and 20 from patients on insulin therapy. The bits were taken, processed, embedded and stained with Hemotoxylin and Eosin, then examined under light microscope. RESULTS: Gross changes observed were increase in weight, diameter, area, circumference and most commonly observed microscopic changes were increased syncytial knots, chorangiosis, villous fibrosis, fibrinoid deposits in Gestational Diabetes Placenta on both diet and insulin therapy when compared to normal group. CONCLUSION: The gross and microscopic changes were marked in insulin therapy group when compared to diet therapy group, hence good glycemic control is needed. This study highlights the pathophysiological areas of clinical research for further study on disease modifying interventions.
