Abstract

INTRODUCTION: Papillary thyroid carcinoma diagnosis is based on presence of papillae and nuclear features. The identification of these features remains difficult at times on histology alone, because of the focal presence of delicate papillae and nuclear features in benign lesions like multi nodular goitre. Also, the nuclear features of borderline lesions like NIFTP tumours are similar to papillary thyroid carcinoma. AIM OF THE STUDY: To evaluate the usefulness of CD56 IHC marker for diagnostic purpose and also to study the effectiveness of CD56 in distinguishing papillary thyroid carcinoma from benign and borderline lesions. MATERIALS AND METHODS: The IHC expression of CD56 was evaluated in 106 thyroid lesions which includes 69 cases of papillary thyroid carcinomas, 27 cases of benign papillary hyperplasia lesions and 10 cases of NIFTP tumours. RESULTS: IHC expression of CD56 was Negative in 90% of PTC tumours, 7% of benign papillary hyperplasia lesions and 20% of NIFTP tumours. This shows that, CD56 is downregulated and negative in most cases of papillary thyroid carcinomas. In our study CD56, as a negative marker, gives sensitivity of 96.88%, specificity of 78.12%, positive predictive value 89.86%, negative predictive value 92.59% and significant P value of 0.002 on comparing PTC and benign PH. CONCLUSION: CD56 is an useful marker to aid in the diagnosis of papillary thyroid carcinoma and is also helpful in differentiating papillary thyroid carcinoma from benign papillary hyperplasia lesions.
