Abstract

INTRODUCTION: The National Institute on Aging NIH, U.S. in its publication “A Global Perspective - Why Population Aging Matters” (2007) recorded that geriatric population will soon out-number children under 5 years, with 2/3rd of the former concentrated in the developing countries; geriatric anemia poses an increased risk for mortality in patients with CCF and CKD. As limited data available on its prevalence and etiology, we attempted this study. AIM OF THE STUDY: To study the profile of anemia in elderly patients and expression of JAK2 mutation in unexplained anemia of elderly. METHODS: We evaluated the patients by a Cross-sectional hospital based study at Tirunelveli Medical College. Patients with anemia, as defined by WHO, underwent a prospective standardized history, physical examination, and laboratory measures, with additional studies including bone marrow examination and mutational analysis as indicated. RESULTS: A total of 240 cases, aged 65 years and above were included. Etiologies of anemia in our study were as follows; nutritional anemia 10%, anemia of chronic inflammation 18%, unexplained anemia 21% and anemia of chronic kidney disease 51%. Those with unexplained anemia further showed 8.8% hypoandrogenism and 0.8% subclinical hypothyroidism with 11.3% of still unknown etiology. Statistical significant correlation seen between serum ferritin and MCV in patients with IDA (p=0.036); between Hb and eGFR (p=0.007). CONCLUSIONS: A comprehensive approach towards elderly anemia would unveil its etiologies. Nevertheless, further studies are needed to explore the root causes like stem cell senescence and early MDS in unexplained anemia
