Abstract

INTRODUCTION: Gastric carcinoma is the fourth most common diagnosed carcinoma worldwide. Gastric carcinoma is more aggressive tumor with high death rate -700000 per year accounting for the second common cause of cancer related death worldwide. Gastric carcinoma occurs more commonly in males. It is a disease occurring in middle age and elderly, having peak incidence in people older than 60 years. There is steady decline in the incidence worldwide, but new cases are increasing because of ageing population. Gastric carcinoma is the most common malignancy in Asia accounting for 18% of all malignancy. 56% of malignancies in korea and Japan are gastric carcinoma. AIMS AND OBJECTIVES: 1. To study the incidence and distribution of gastric carcinoma in patients admitted in Tirunelveli medical college hospital during October2016 to May 2019. 2. To study the various clinicopathological factors of gastric carcinoma including age of incidence, sex preponderance, tumor location, tumor size, gross appearance, depth of infiltration, lympho vascular invasion, perineural invasion, tumor type, tumor grade, lymph node invasion and metastasis. 3. To determine the immunohistochemical expression and scoring of HER2NEU in gastric carcinoma. 4. To study the association HER2NEU in gastric carcinoma with known factors like age, sex, tumor size, tumor location, tumor histological type, grade, depth of infiltration, lympho vascular& perineural invasion, grading and tumor stage. MATERIALS AND METHODS: Study design: Cross sectional study. Study setting: Department of pathology, Tirunelveli Medical College, Tirunelveli. Study subjects: Histological grading and HER2NEU scoring in gastric carcinoma. Inclusion criteria: Gastrectomy specimen received in the Department of Pathology, Tirunelveli Medical College. Gastric carcinoma -adenocarcinoma. Exclusion criteria: Endoscopy biopsy. Malignancy of stomach other than adenocarcinoma. Study period: From October 2016 to May 2019. Sample size: 70 specimen. RESULTS: In this study 70 gastrectomy specimens received in the Department of Pathology, Tirunelveli Medical College are included. Their relation with age distribution , sex distribution, type of surgery, tumor site, tumor type, histological grading ,microscopic extension, proximal and distal margin involvement, lymphovascular and perineural invasion and lymph node involvement are studied and tabulated. Immunohistochemistry for HER2NEU is done for all Gastric carcinoma cases. CONCLUSION: In this study all advanced Gastric carcinoma cases received in the Department of Pathology, Tirunelveli medical college in the period of October 2016 – May 2019 (4 years) are included. Most of the tumors are located in pyloric region . Most common type is intestinal type adenocarcinoma with moderately differentiated grade. Of 70 cases were studied with IHC for HER2NEU expression. 25 cases (36%) showed positive HER2NEU expression (3+). Nowadays along with other modes of treatment for Gastric carcinoma, targeted therapy with monoclonal antibodies against HER2NEU receptor has helped in increasing the patient survival. Hence evaluating HER2NEU expression in Gastric carcinoma has gained importance.
