Abstract

BACKGROUND: The prevalence of coronary artery disease is on the rise with an increase in sedentary life style. Platelet activation in response to plaque disruption, resulting in occlusive thrombosis is the paradigm for acute coronary syndrome. Platelet indices such as platelet count(PC), mean platelet volume(MPV), platelet distribution width(PDW), platelet large cell ratio(P-LCR) and plateletcrit(PCT) are determinants of platelet function. AIMS AND OBJECTIVES: 1. To assess the role of platelet indices and histogram in hospitalised patients with acute coronary syndrome and to find out the correlation. 2. To study the changes in platelet indices and histogram in patients with acute coronary syndrome and to assess their usefulness in predicting the risk of coronary events. MATERIAL METHODS: This is a prospective and comparative study conducted in Institute of Pathology for a period of 1.5 years which included 218 cases of acute coronary syndrome and 54 controls. Blood samples were obtained in EDTA anticoagulated tube and platelet indices were measured using automated blood cell analyser SYSMEX XP 100 and then analysed. RESULTS: Platelet indices (MPV, PDW, P-LCR) were significantly higher in cases in comparison to controls. PC and PCT were found to be lower in cases with no significant statistical difference. Platelet histogram had no significant difference between two groups. The use of platelet indices in laboratory routines could be an important complement in the assessment and followup of cardiac patients especially in remote centres and in centres where cardiac serum markers are not readily available. CONCLUSION: The results of the present study appear to confirm and substantiate that an increased platelet indices like mean platelet volume(MPV), platelet distribution width(PDW) and platelet large cell ratio(P-LCR) contributes to the prothrombotic state in Acute coronary syndrome(ACS) and that larger platelets may also play a specific role in ACS. Hence we conclude that the use of platelet indices in laboratory routines could be an important complement in the assessment and follow up of cardiac patients especially in remote centres and in centres where cardiac serum markers are not readily available since these indices are easily provided by automated equipment which is simple and cost effective.
