Abstract

BACKGROUND: The onset of Type II diabetes mellitus is often insidious and silent. Undiagnosed and untreated chronic hyperglycemia is responsible for the relatively high prevalence of diabetic retinopathy and diabetic nephropathy in newly diagnosed T2DM. AIMS AND OBJECTIVES: 1. To study the prevalence of retinopathy and nephropathy in newly diagnosed type 2 diabetes mellitus patients 2. To study the relationship of development of nephropathy and retinopathy with various risk factors associated with type 2 diabetes mellitus like BMI, lipid profile, uric acid. MATERIALS AND METHODS: A prospective cross sectional study was conducted in the department of General Medicine Tirunelveli Medical College Hospital, Tirunelveli. A total of 100 newly diagnosed T2DM patients were included in the study. Detailed history, clinical examination, fundus examination and relevant investigations were done to diagnose diabetic retinopathy and diabetic nephropathy. RESULTS: Out of the total 100 patients in this study 58 were males and 42 females. In our study mean age of our patients were 40 years. Diabetic retinopathy was present in 6% of patients and diabetic nephropathy was present in 24% of patients. In our study lipid profile is abnormal in 56% of patients with newly diagnosed T2DM. Subjects with HbA1C more than 7.5% had more microvascular complications like diabetic retinopathy and nephropathy. CONCLUSION: A long duration of asymptomatic hyperglycemia in T2DM patients is responsible for diabetic retinopathy and nephropathy at the time of diagnosis. So a high prevalence of diabetic retinopathy and nephropathy at the time of diagnosis in our study reconfirms that assessment for these complications must be done at the time of diagnosis in all patients. Once complications develop in addition to strict control of hyperglycemia steps must be taken to prevent or delay further progression of these complications.
