Abstract

OBJECTIVES: Objective of the present study was to describe clinical features, risk factors, microbiological aspects, catheter related infections and outcome of high risk febrile neutropenia in acute leukemia patients during intensive phase of chemotherapy. PATIENTS AND METHODS: 115 high risk febrile neutropenia episodes were prospectively studied from January 2011-December 2011. Uniform antimicrobial policy was followed. RESULTS: Lung was the most common clinical focus of infection and was associated with detrimental outcome. Microbiological documentation was possible in 38 episodes (33%). Gram negative bacteria constituted 58.3% of bacterial isolates and were highly sensitive to amikacin and cefoperazone-sulbactum combination. There were no vancomycin resistant Enterococcus isolates. Success rate of first line empiric antifungal agent was 70%. CONCLUSION: In a resource constraint setting, it may not be prudent to use carbapenem as a routine first line empiric antibiotic in febrile neutropenia, rather carbapenem and glycopedtide agents should be used judiciously.
