Abstract
To assess the effectiveness of foot massage on pain and anxiety among cancer patients in selected palliative care centre at Madurai was conducted as a partial fulfillment of the requirement for the award of the degree of Master of Science in nursing by R.Sumathi from college of nursing, Madurai Under the Tamil Nadu Dr. M.G.R Medical University Chennai, April 2012. INTRODUCTION: Cancer is one of the most occurring chronic diseases in the world affecting 9.6 million people died of cancer in 2006 (WHO 2010). The causes for cancer are complex. It is difficult to assess the specific cause for a specific cancer. The economic burden of cancer is enormous. If a survivor’s illness affects his or her ability to work, less income goes to the individual and family. Pain poses a great challenge. This type of pain often affects a person’s life in many ways. It can change someone’s personality, ability to function and quality of life. Many patients experience a number of episodes of breakthrough pain each day. Anxiety is also common for the cancer patient’s populations. Anxiety has both physiological and psychological components. Anxiety symptoms affect 13% to 16 % of patients with cancer .Foot massage is a complimentary therapy and multidisciplinary for cancer pain and anxiety management massage acts like an analgesic and inhibits those pain signals from being transmitted to the brain. It is the manipulation of superficial and deeper layers of muscle and connective tissue to enhance function and in the healing process and promote relaxation and well being. OBJECTIVES: 1. To assess the level of pain and anxiety among cancer patients. 2. To evaluate the effectiveness of foot massage to reduce pain and anxiety among cancer patients in experimental and control group. 3. To compare the pre test and post test level of Pain score and Anxiety score among cancer patients in experimental group and control group. 4. To associate the level of pain and anxiety among cancer patients in experimental and control group with selected demographic variable. CONCEPTUAL FRAME WORK: The conceptual frame work adopted for the present study was based on the CIPP MODEL. It was created by Daniel L.Stufflebeam. It is an acronym that stands for context evaluation Input evaluation process evaluations and product evaluations. METHODOLOGY: The tool developed and used for the data was interview schedule, numerical rating scale and Hamilton anxiety scale was used to assess the pain and anxiety level. The content validity of the tool was established by nursing experts the reliability of the tool was established by split half method. Calculated correlation coefficient "r" value is 0.81 was high. The pilot study was conducted in the Annanagar and the study was found to be feasible. The main study was conducted at Asirvatham palliative care centre at Madurai. Among 60 patients 30 patients in experimental group and 30 patients in control group were selected by convenient sampling method, based on sample selection criteria. First, data was collected from the control group and then followed by experimental group. Pretest on pain and anxiety level assessed for both groups. The intervention through foot massage was given 20 minutes every day for five consecutive days among experimental group. Post test of the pain and anxiety was measured on 5th day the data gathered were analyzed and interpreted in terms of objectives by using SPSS package. A probability of less than 0.05 was used to accept the hypothesis. FINDINGS: There was a significant difference between the mean pain score before and after foot massage in experimental and control group t=13.22(p=0.001), t=17.59(p=0.001) There was a significant difference between the mean anxiety score before and after foot massage in experimental and control group t=17.59, p=0.001 and t=18.41, p=0.001. CONCLUSION: The study concludes that the cancer patient in the experimental group has reduction of pain and anxiety after foot massage. So in addition to the pharmacological treatment foot massage can be used for managing pain and anxiety among cancer patients.
