Abstract

The aim of the study is to evaluate the therapeutic efficacy of Nagarasingathi chooranam (internal medicine) and Maeni thean (Internal medicine) in Swasa kasam. ❖ The form B and study protocol has been approved by Institutional animal ethical committee and Institutional ethical committee. ❖ The raw drugs were authenticated by concerned department and the medicine was prepared in the Gunapadam laboratory of National Institute of Siddha. ❖ Toxicity studies were done in the pharmacological laboratory of National Institute of Siddha. ❖ Biochemical analysis (Qualitative analysis) was done in the Biochemistry lab of NIS and Quantitative analysis was done in IIT, Chennai. ❖ For the clinical study, 40 cases were selected based on the inclusion and exclusion criteria. Out of this, 30 cases were treated in OPD and 10 cases were treated in IPD of Ayothidoss pandithar hospital of NIS, Chennai. ❖ A day before starting the trial drug treatment, purgation was given to correct the elevated mukkutram. ❖ The clinical trial was conducted in 40 patients of Swasa kasam with the trial drug Nagarasingathi chooranam(internal medicine) at the dose of 2gm thrice a day with adjuvant of honey and Maeni thean (Internal medicine) at the dose of 5m1 thrice a day for 48 days. ❖ During the study period, there were no adverse reactions. ❖ The trial drugs were found to play the major role to correct the deranged three humours, thereby correcting Pranan, Abanan, Udhanan, Kirugaran, Devathathan Vayus, Pitham such as Anal Pitham and the vitiated kabam is restored to the normal. ❖ Blood and urine Investigations were carried out before and after treatment and data were recorded in the proforma. ❖ Radiological investigations (Chest X-ray PA view) and ECG were also done before treatment. ❖ Clinical assessments Progress were done once in 7 days for OPD patients and daily for IPD patients. ❖ Toxicity study reveals that the trial drugs is safe even in higher dosage of Nagarasingathi chooranam — 1080 mg/animal and Maeni thean — 2.7m1/animal in albino rats as per WHO guideline 1993. ❖ As per objective parameters (PEFR) Out of 40 cases, 65% of cases had clinically good improvement, 30% of cases had clinically moderate improvement and 5% of cases had clinically mild improvement. ❖ Out of 40 cases, Clinical results were found to be Good in 85% of cases, Fair results were found in 12.5% of cases and Poor results were found in 2.5% of cases. CONCLUSION: ❖ Toxicity study reveals that the trial drugs is safe even in higher dosage of Nagarasingathi chooranam — 1080 mg/animal and Maeni thean — 2.7m1/animal in albino rats as per WHO guideline 1993. ❖ The results of the clinical trial indicate that the trial drugs are clinically effective. No adverse effects were reported during the course of treatment. ❖ The method of preparation is easy and the cost is comparatively economical. ❖ Clinical results were found to be Good in 85% of cases, Fair results were found in 12.5% of cases and Poor results were found in 2.5% of cases. ❖ Because of the encouraging results clinically, the study may be undertaken with same medicine in a large number of cases and it may throw new lights for the treatment of Swasa kasam.
