Abstract

ABSTRACT BACKGROUND: Breast cancer(10.9%) is most common worldwide next to lung cancer. Among the risk factors identified for primary breast carcinoma, environmental and life style factors are important. Cancer patients and survivors suffer from psychological problems like adjustment disorder, major depressive disorder, generalized anxiety disorder and phobic disorders. Majority of the suicide victims of carcinoma breast have depression. It increases the burden of distress and dysfunction. Depression in these patients increases morbidity, length of hospital stay and overall disability. This decreases the motivation and compliance to chemotherapy. AIM OF THE STUDY: To study the prevalence of psychiatric morbidities in newly diagnosed carcinoma breast in females and to study effect of stressful life events and quality of life and their impact on the outcome of carcinoma breast. OBJECTIVES To study the psychiatric morbidity in newly diagnosed carcinoma breast in females. To assess the relationship between psychiatric morbidity and psychosocial stressors.To assess the relationship between psychological morbidity in carcinoma breast in females with quality of life. To understand the relationship between psychological morbidities with socio demographic and illness related variables METHODOLOGY: The Study is conducted in the patients suffering from carcinoma breast who are admitted for medical management in the Medical Oncology Department, Government Rajaji Hospital, Madurai. The period of study was six months. The study was done on 60 consecutive patients who were admitted as in patients in the department of medical oncology. The patients were subjected to several scales to assess their psychiatric morbidities like MINI International Neuropsychiatric Interview, HADS- Anxiety, Depression scale, Presumptive Stressful Life Events scale, Body Image Scale, Beck's Suicidal Ideation Scale. RESULTS Psychiatric morbidities are more prevalent among carcinoma breast patients. Younger age patients are suffering with more psychiatric morbidities. Patients with psychiatric morbidities experienced more stressful life events. There is no significant relationship between the staging of carcinoma and psychiatric morbidities Significant number of patients developed psychiatric disorder before the onset of symptoms of breast carcinoma. Patients with psychiatric morbidities have significant body image disturbances. Body image disturbances worsen the quality of life. CONCLUSIONS: Thus by identifying the underlying psychiatric morbidities in newly diagnosed carcinoma breast patients will help the treating physician to look for underlying factors contributing to depression and anxiety, and hence improve the overall outcome and quality of life in these patients. By early identification and referral, the potential dangers like suicide risks could be prevented.This will improve the overall outcome of patients with carcinoma breast
